Essential duties include but are not limited to all or some of the following: Renders public health nursing services in the field of maternal, child and adult health; conducts home visits for birthing and postpartum people and families; interprets treatment plans and assists clients in understanding; provides health education related to nutritional needs, hygiene and safety measures; guides clients after delivery regarding postpartum and newborn care and explains how to access emergency medical assistance; assesses the health, safety, and development of newborn infants and supports clients within infant care. Renders direct nursing services to vulnerable populations including children, adults, and older adults with disabilities, special health care needs, clients with chronic illness, mental health, and/or substance use disorder; assists priority populations with access to medical care as needed; links/refers cases to appropriate human services, community, and safety net resources; maintains supervision of cases and keeps appropriate records of nursing and other services rendered. Provides care coordination of complex medical and health needs of these population including accessing and coordinating services identified in the individualized service plan. Prepares and maintains individual client records in individual charts; uses required forms for referrals, consents, and narrative and statistical records; prepares various statistical information and reports related to client outcomes. Job Description The General Surgery Nurse Practitioner complements and assists the work of the attending faculty, surgical fellow, and surgical residents to provide integrative inpatient and outpatient care for the surgery services, which include acute care, gastrointestinal, bariatric, colorectal, hepatopancreatobiliary, surgical oncology, breast, plastic, and endocrine surgery. The General Surgery Nurse Practitioner serves as a high-level professional, whose activities span the full range of care of adult surgical patients, taking responsibility for the safe delivery of care under the supervision of attending physicians and residents. Full assumption of these responsibilities will follow a training period, during which time the General Surgery Nurse Practitioner will be monitored by a physician supervisor to determine that knowledge and skills have reached the appropriate level. Job Description Reporting to the Director of Operations for Hematology, Blood and Marrow Transplant and Cellular Therapies (HBC), the Director of Nursing, HBC is a clinical leader and contributes to the development of UCSF Health and the Cancer Center’s strategic goals and objectives as well as overall standards of nursing practice. The Director of Nursing is a member of the HBC management team and works within the framework of the mission and values established by UCSF Health. The Director of Nursing oversees all adult HBC faculty practices clinical operations, systems and processes, providing direct and indirect supervision of administrative and clinical staff in multiple departments including; malignant hematology, nonmalignant hematology, blood & marrow transplant and cellular therapies. The Director of Nursing ensures exemplary provision of care to research and standard-of-care patients in both inpatient and ambulatory settings. Working in a matrixed environment, the Director of Nursing partners to develop systems for integration with the broader health system (e.g. infusion, radiation oncology, inpatient services, pharmacy, imaging, labs, UC satellite locations). Job Description Interested in a dynamic, autonomous practice setting, working with a highly skilled and collaborative APP team? Consider joining our ambulatory care Cancer Center Infusion APP team. The Cancer Center Infusion APP team is responsible for overseeing the care of 50-75 patients per day (depending on size of unit). We cover five Infusion centers providing treatment for Oncology and non-Oncology patients. The Infusion APPs triage multiple issues in real time and ensure continuity of care through collaboration and communication with physicians/surgeons/APP colleagues as well as other clinical and administrative staff. Ultimately, we ensure timely patient care and treatment to meet goals of care, optimize the patient experience and ensure good patient outcomes. In addition to our infusion areas, we cover our three bed Cancer Center Acute Care (CACC) where we manage higher acuity Oncology patients to avoid unnecessary emergency room visits, hospital admissions and facilitate emergency services and admission when necessary.
