Nurse Practitioner- Palliative/Hospice Care Full Job Description Primary Duties and Responsibilities The Home Health- Nurse Practitioner is responsible for assisting in the expedient multidisciplinary care of patients for Hoag Hospital and its affiliates, coordinating seamless patient care from admission through discharge in the Home Health setting. Providing supplemental medical care to patients with a focus on quality and patient experience, the Nurse Practitioner can perform history and physical exams, diagnoses and treats medical problems, orders and interprets diagnostic and laboratory tests, performs minor procedures and facilities the discharge process. Job Summary: Selects and administers intravenous, spinal and other anesthetics prior to and during surgical or medical procedures, as a member of the anesthesia care team and in accordance with Kaiser Permanente policies and procedures.
