A Brief Overview Responsible for assessment, planning, implementation and evaluation of direct care to assigned patients with simple to complex health care needs in inpatient, outpatient and procedural areas. What You Will Do Delivers patient care in accordance with the ANA Scope and Standards of Care and Practice, Patient Bill of Rights and the ANA Code of Ethics for Nurses. Delivers patient care in accordance with specialty specific standards (e.g. Oncology Nurses Association, American Association of Critical Care Nurses) as appropriate Assesses, plans, implements and evaluates the patient plan of care according to changing health care needs. Assesses, plans, implements and evaluates patient teaching using appropriate education resources and evaluates the effectiveness of the teaching. Models effective leadership skills through role modeling, mentoring and effective communication. Assumes charge nurse responsibilities as assigned. Directly delegates and supervises the practice of ancillary staff (LPN’s, PCA’s, Tech’s, Unit Secretaries, etc.) and is knowledgeable of the tenets of the NJ Scope of Practice guidelines. Supports a professional practice environment by functioning as a role model and resource person and also functions as a preceptor for nursing staff and participates in the orientation of other unit employees. Participates in unit-based and organization-wide activities, committees and community activities as appropriate Identifies own learning needs with respect to nursing practice and pursues educational support to meet these needs. Routinely utilizes Evidence Based Practice literature, policy and procedure manuals and other available reference materials to ensure consistency and accuracy in the delivery of care. Supervise and assist with patient monitoring during stress testing, including IV placement, vital signs, and ECG monitoring. Assist with preliminary reporting and quality assurance as needed. This is a full time position located in Bay Ridge Brooklyn. The schedule is Mon/Wed/Fri 9:00AM-5:00PM and Tues/Thurs 10:00AM – 7:00PM. One Saturday a month required. Description The Medical Assistant I provides clinical office support to physicians and surgeons and performs patient care and administrative duties necessary to deliver quality patient care. Responsibilities Fulfills patient care responsibilities as assigned which may include: performing venipuncture and/or EKGs, checking schedules and organizing patient flow; accompanying patients to exam/procedure room, assisting patients as needed with walking, transferring and dressing, as well as collecting and processing specimens and point of care testing, preparing for exam, etc. Assists physicians with various procedures, takes vital signs, relays instructions to patients and or families; answers calls and provides pertinent information. Fulfills clerical responsibilities as assigned which may include: sending and receiving patient medical records; obtaining lab/x-ray reports, hospital notes, referral information, etc. Completing forms and requisitions as needed, managing charts to ensure information is completed and filed appropriately. These responsibilities may be carried out using the electronic medical record (EMR) system or manually as necessary. Enters orders into the electronic medical record (EMR) system or prepares prescription refill requests on behalf of the physician for the review and approval by the ordering physician. Inventories, orders or re-stocks medical supplies as applicable. Provides set-up of examination room and instruments in accordance with proper sterilization techniques established by hospital and infection control policies. Maintains logging system as required for refrigeration of medications, point of care testing, sterilization process, crash cart, point of care testing or others as needed. Checks expiration dates on all medications and medical supplies disposing appropriately as per institution policy. Maintains a safe, secure, and healthy work environment by following standards and procedures; complies with legal regulations. This position will cover hospital locations in Essex County and support the Northern Hospital Division team during on-call hours. Responsibilities: Complete evaluations, admissions, and discharges within our Northern Hospital Division as needed. Medicare-eligibility review for new referrals and determining if patients meet criteria for GIP-level hospice. Follow up on current GIP hospice patients to ensure needs are met during on call hours. Reinforce hospice recommendations made by RN Case Managers and make adjustments as needed. Collaborate with doctors, nurses, social workers, patients, families, and our hospice staff to promote exceptional end-of-life care. Be available to answer any questions and provide education as needed regarding hospice services and philosophy. Description As an integral part of the clinical care team, the Patient Care Associate is responsible for patient care and support activities promoting patient focused care within the Specialty Service. The Patient Care Associate supports the registered nurse in patient care activities, such as: assisting patient with activities of daily living, collecting specimens and performing procedures; maintaining a safe, clean environment; and other duties as needed. Responsibilities A. Clinical/Technical/Service • Demonstrates the ability to perform clinical/technical/service/administrative tasks • Demonstrates the knowledge and skills necessary to provide care, based on physical, psychosocial, educational, safety, and related criteria, appropriate to the age of the patients served in assigned area. • Performs and records patient’s vital signs, such as blood pressure, pulse, temperature and respirations. Documents intake and output and other data as assigned • Obtains patient’s rating on pain scales as a fifth vital sign in all areas except Pediatrics • Performs clinical procedures, such as specimen collection including blood glucose, venipuncture, EKG, application of compression devices, post-mortem care, and maintenance of skin integrity. • Sets up for selected procedures and assists RN’s accordingly. • Assists patient with activities of daily living, such as feeding, bathing and/or toileting. Assists the patient in room maintenance, such as bed-stripping and trash removed, when necessary • Performs on-to-one observation or close observation status of patients, e.g. safety, elopement risk, or suicide prevention. • Maintains “isolation status” for patients under direction of an RN and according to set procedures • Performs scheduled hourly care checks/ rounding on patients with emphasis on caring for toileting needs, comfort, and positioning and pain awareness.
