Swedish Medical Group is seeking an experienced Pulmonary nurse practitioner to join a highly collaborative, fast-paced outpatient practice in Seattle, Washington. In this role, you’ll collaborate closely with pulmonologists to deliver exceptional respiratory care to a diverse patient population across 2 outpatient clinics in First Hill and Issaquah. Primary Tasks : Caseload Management: Reviewing a roster of high-risk patients (often ~100+). Field Visits: Traveling to dialysis centers or nephrology offices to meet patients during their treatment/appointment. Assessments: Conducting Comprehensive Health Evaluations (CHEs)—head-to-toe assessments covering medical, social, and emotional needs. Education & Coordination: Teaching patients about comorbidities (diabetes, heart failure), reconciling medications, and arranging follow-up care to prevent hospital readmissions. Documentation: Heavy focus on documentation for risk adjustment and care plans (often completed from home). Outcome Focus: Reducing hospitalizations and managing overall health long-term . Care coordination along with Nurse Practitioner and Care Coordinator.
