PROGRAM LIAISON FOR INPATIENT REHABILITATION: Assumes responsibility for the implementation of each individual patient program. Assists the patient/family to become adequately oriented to their program. Enables the patient's program to proceed in an orderly, purposeful, and goal directed manner. POPULATION HEALTH RN CASE MANAGER: Serves as a clinical resource for patients and families enrolled in Population Health Management programs such as Enhanced Care Management (ECM), Chronic Care Management, Transitional Care Management with goal of improving health outcomes, reducing unnecessary healthcare utilization and addressing Social Determinants of Health (SDOH) in partnership with a multidisciplinary team to include Primary Care Provider, Community Care Coordinator, Pharmacist, Medical Assistant, etc. Additional Demonstrates the knowledge and skills necessary to provide care and services appropriate to the population served on the assigned unit or work area. Experience working with Medi-Cal populations and individuals with complex medical, behavioral health, and social needs. Experience supporting populations experiencing homelessness, justice involvement, serious mental illness (SMI), substance use disorders (SUD), or multiple chronic conditions. Experience providing clinical guidance, consultation, mentoring, or supervision to interdisciplinary care teams preferred. Strong understanding of care coordination, population health, and interdisciplinary team-based care. Excellent communication, organizational, and problem-solving skills. POSITION DESCRIPTION Responsible for timely and accurate completion of both the RAI process and care management process from admission to discharge in accordance with company policy and procedures, and Federal, State and Certification guidelines, and all other entities as appropriate- Minimum Data Set, discharge and admission tracking, etc. With direction from the Director of Nursing and VP of Clinical Reimbursement, may coordinate information systems operations and education for the clinical department. Principal Responsibilities • Conduct the daily nursing functions in accordance with Company, State, Federal and local rules, regulations, and guidelines. • Admit, transfer and discharge residents with documentation regarding care and service provided. • Participate in the orientation of new residents/family members to the facility. • Participate in facility surveys (inspections) as scheduled. • Ensures delivery of high-quality, positive clinical outcomes, and resident/family satisfaction during shift. • Serves as a resource to all staff within the facility regarding the quality of services provided. • Ability to administer medications and treatment timely and according to facility policy; Demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice Act. • Effectively manages nursing personnel on shift to limit use of overtime and agency personnel. • Assesses the work performance of nursing staff as it relates to unit standards of care and goals. • Conducts unit huddles to promote communication programs and provides updates to DON. • Participates in shift-to-shift communication between incoming and outgoing nursing staff. • Coordinates shift assignments, resident assignments, rest and meal break for Certified Nurse’s Aides. • Collaborates with other departments to provide timely effective care consistent with individual’s needs, choices, and preferences. • Assumes nursing administrative responsibilities of the center for weekend and holiday coverage as requested. • Stays and works beyond scheduled shift if needed to meet state staffing requirements and/or needs of residents. • Ensures timely and accurate compliance of the MDS process and that it reflects the resident’s plan of care. • Ensures Point Click Care (PCC) is utilized according the company process. • Ensures that physician orders are followed as prescribed. General Responsibilities • Attends and participates in scheduled training, educational classes, and meetings to maintain current certification as applicable; Attends and participates in in-service training as mandated by regulatory agencies and company policy. • Adheres to work schedules in completing and performing assigned tasks; completes all assigned tasks in a timely manner as instructed by the direct supervisor. • Meets acceptable center attendance requirements; notifies facility before late arrival to or absence from work according to established center policy. • Cooperates with other facility personnel to ensure that services can be adequately maintained to meet the needs of residents; creates and maintains an atmosphere of warmth, personal interest, and positive emphasis, as well as a calm environment.

QUALIFICATIONS Current LVN/LPN license in practicing state. Current BLS/CPR certification or willingness to acquire one. Experience in nursing management is preferred but not required. RESPONSIBILITIES Follows established procedure for charting and incident reporting. Follows company procedures for narcotic counting, pharmacy orders, and restraint reduction. Documents any change in patient's conditions including but not limited to nutritional problems, infections, skin conditions, weight loss or any other noticeable changes. Duties of Hospice Registered Nurse Include: Completes an initial, comprehensive and ongoing comprehensive assessment of patient and family to determine hospice needs. Provides a complete physical assessment and history of current and previous illness. Conducts home visits and assesses/evaluates patient's status. Initiates the plan of care and makes necessary revisions as patient status and needs change. Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process. Administers medications and treatments as prescribed by the physician. Attends and participates in Interdisciplinary Group Meetings. Completes, maintains and submits accurate and relevant clinical notes regarding patient's condition and care given. Supervises ancillary personnel and delegates responsibilities when required. Qualifications: Graduate of accredited school of nursing Current RN license in California Current CPR Certificate Registered nurses shall have a minimum of (a) one year of experience as a professional nurse within the last three years OR (b) have a baccalaureate degree in nursing from a program accredited by the National League for Nursing and current RN license. Minimum of two years experience, at least one of which is in the area of public, home care, or hospice nursing. Must have a valid CA driver license and an automobile that is insured in accordance with the requirements of state of California and is in good working order.
