ESSENTIAL JOB FUNCTIONS Performs physical examinations and preventive health measures within prescribed guidelines. Orders, interprets, and evaluates diagnostic tests to identify and assess patients’ clinical problems and health care needs. Records physical findings, and formulates plan and prognosis, based on patient’s condition. Provides written home-going instructions. Collaborates with Physician, behavioral health professionals and other health professionals to prepare comprehensive patient care plan as necessary. Prescribes or recommends medications or other forms of treatment such as physical therapy, inhalation therapy, or related therapeutic procedures. Refers patients to Physician for consultation or to specialized health resources for treatment. Maintains medical records using (In some health centers it may be using Electronic Medical Record software). Educates and coaches nursing staff on best nursing practices. Interacts with clients to assist in gaining insight, defining goals, and planning action to achieve effective personal, social, educational, and vocational development and adjustment. In additional to individual counseling, this position may lead psychotherapy groups, seminars, and provide couples and/or family sessions, when appropriate to the site contract Provide psychotherapy to individuals of all ages. Interviews clients to gather and interpret social, personal, environmental, and health information. Evaluate and treat problems within their scope of practice. Coordinates and plans programs and activities to meet clients social and emotional needs. Facilitate cognitive, affective, and behavioral health cases, including short term crisis resolution and referrals to community resources. Establish and maintain a relationship of mutual respect, acceptance, and trust. Evaluate the effectiveness of treatment services provided to the client. Identify appropriate resources and assessment instruments, as needed. Document findings in client records according to established protocol. Keep case load as required and ensures case documentation is up-to-date and reflective of services. Identify psychological, emotional, or behavioral issues and diagnose disorders, using information obtained from interviews, tests, records, and reference materials. Use a variety of treatment methods, such as psychotherapy, behavior modification, stress reduction therapy, psychodrama.
