Job Description Summary Make a difference with our Infusion team! ESSENTIAL FUNCTIONS Assesses patients on assigned units based on policy. Assessments to include review of the treatment plan, determination of the appropriate level of care, assessment of discharge needs and initiation of the discharge plan process. Collaborates with the social worker regarding patients complex social and discharge needs. Develops and implements a discharge plan proactively through collaboration with physicians, patients, families, multidisciplinary team members and other external caregivers as applicable, to facilitate a seamless transition from one level of care to another across the health care continuum. Attends daily rounds on assigned unit with team members to ensure that the multidisciplinary plan of care is consistent with the patient's clinical course, continuing care needs and covered services as evidenced by documentation of care needs and interventions. Reassesses continually the plan of care and discharge needs of the patient and collaborates with the members of the multidisciplinary team to modify the plans as needed based on the patient's changing needs as evidenced by weekly documentation in the progress notes. Maintains a working knowledge of behavioral responses to illnesses and other areas (community resources, payer requirements) to facilitate the patient's movement along the health care continuum. Maintains appropriate documentation in the medical record and in computer systems as required by policy or departmental practice. Makes all appropriate referrals needed to implement the discharge plan, which include LTACH, post acute facilities and home care on a timely basis. Follows up to ensure that the services needed by patients/families are in place prior to discharge. Identifies, develops and implements strategies to reduce length of stay and resource consumption. Handles daily assessment of patient hospital course to proactively identify issues with completion and reporting of diagnostic testing, completion of treatment appropriate for the acute episode of care, facilitation of appropriate level of care and appropriate utilization of resources, discussing estimated LOS for diagnosis and communicating the discharge plan. Works collaboratively with the multidisciplinary team when issues are identified, to facilitate appropriate interventions. Collects accurate data on the variances for analysis on daily reports. Identifies accurately issues that have a negative impact on patient care and reports these issues timely to the responsible entity. Identifies accurately and advises appropriate manager/director when physician or patient issues require legal, ethical or administrative interventions. Participates in performance improvement activities in the management of patient care as demonstrated in monthly PI reports. Performs other duties as assigned. The responsibilities of this job include, but are not limited to the following: Locating patients, obtaining accurate vital signs by visual and auditory measures, triaging patients, assessing patients’ medical condition, and preparing patients for physicals and medical treatment by Physicians and Nurses; Understanding and using the electronic medical record system; Ordering lab, x-ray, and medical supplies as directed; Notifying the Nurse and/or Physician when the patient is ready for exam while communicating symptoms and relaying questions about lab or x-ray needs; Assisting Nurses and Physicians with various forms of patient treatment and care; Handling Physician and Nurse requests with respect and in a timely manner; Performing lab duties as assigned; Performing phlebotomy procedures; Sterilizing and setting up instruments; Receiving and storing supplies; restocking treatment rooms as needed; Reporting to the Charge Nurse before leaving at the end of the shift; Following ancillary guidelines as deemed necessary by the Charge Nurse, Nurse Supervisor, DMS, and Physician; Adhering to established policies and procedures; Maintaining cleanliness of the treatment area and completing biological cleanup in accordance with OSHA guidelines and other maintenance assignments as directed; Washing, drying, folding, and restocking laundry; maintaining the cleanliness of the washing machine and dryer lint trap; Attending staff meetings as scheduled by the Nurse Supervisor or DMS; Operating, using, and maintaining medical and office equipment as trained; Completing assigned checklists and Policy Manager tasks within the assigned shift; Being available to assist as needed, as breaks and mealtimes may be interrupted at any time to provide necessary patient care or to maintain center operations; Fostering teamwork and ensuring a positive and professional atmosphere; Providing positive, warm, and friendly service in all interactions; Understanding and performing all duties as assigned. Unit Summary: This unit is a 21-bed unit that provides leading- edge, collaborative, and comprehensive care for adult patients with a variety of blood and bone marrow disorders. As a part of the VCU Massey Cancer Center, the largest adult stem cell and bone marrow transplant provider in all of Virginia, we offer the latest FDA- approved CAR-T immunotherapies, clinical trials testing new and promising forms of cellular immunotherapies, as well as all forms of stem cell and bone marrow transplantation. We provide both outpatient and inpatient services in the same facility, and provide a suite of support services that range from helping minimize symptoms and side effects to coping with the cognitive, emotional, and psychological effects of cancer. A typical shift for our nurse includes a team of mixed registered nurses, clinical coordinators, and Patient Care Techs (PCTs) caring for adult patients. We are dedicated to providing a fulfilling and high energy work environment. Our nurses and PCTs have a unique perspective on compassion and a strong belief in the enormous potential of cancer survivorship. Acute Care Pediatrics 12 is a 24-bed pediatric unit. The staff care for patients from a variety of medical specialties including, but not limited to nephrology, neurology, cardiology, pulmonology, gastrologic, and endocrinology. The unit also sees surgical specialties such as trauma, plastics, and general care surgery patients. The unit consists of a combination of general care and step-down patients. Special to ACP 12 are the hematology/oncology, epilepsy monitoring and bone marrow transplant populations. Additionally, some patients are monitored by telemetry in our general care and step-down environments. The Nurse Clinician demonstrates a high level of expertise in the assessment, diagnosis and treatment of the complex patients and their responses to actual or potential health problems. This role assists the health care team with the prevention of illness and injury, maintenance of wellness, and provision of comfort. The Nurse Clinician provides direction, education, leadership and accountability for program development, implementation and evaluation in area of responsibility. This role includes professional development and empowerment of staff as well as management of resources to promote desired outcomes with attention to quality, efficiency and cost.
