The Licensed Practical Nurse in Home Health is responsible for providing quality patient care and performing technical skilled care in the patient's home. Primary Responsibilities: Provides high quality clinical services within the scope of practice, as defined by the state laws governing the practice of nursing, in accordance with the plan of care, and in coordination with other members of the health care team, following all applicable infection control procedures Documents patient visits thoroughly and completely per regulatory and payer requirements in the electronic medical record utilizing the agency's electronic visit verification program (as applicable) Actively participates in the agency's care coordination process including timely reporting patients' needs and changes in condition, attending patient case conference, communicating effectively to the patient, family, physician, other clinicians, agency management staff, and case coordinator, community case managers (as applicable) Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client You'll be rewarded and recognized for your performance in an environment that will challenge you and give you clear direction on what it takes to succeed in your role as well as provide development for other roles you may be interested in. Required Qualifications: Current and unrestricted LPN licensure in state of practice Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation Preferred Qualifications: Current CPR certification or ability to complete within 90 days of hire Home care experience Ability to manage multiple tasks simultaneously Able to work independently Good communication, writing, and organizational skills Ability to work flexible hours as required to meet identified client needs Pay Range $49,151 - $73,726 annual total cash target pay $28.36 - $42.53 per visit point $23.63 - $35.45 hourly rate Annual total cash compensation for this role assumes full-time employment (40 weekly hours) at full productivity and generally follows the range above. Total cash compensation includes earnings from per visit point pay and hourly pay and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role. This role receives two types of compensation depending on the work being performed. When conducting visits, you will be paid per visit point rate compensation. Your per visit pay will be calculated by multiplying your per visit point rate by the productivity points you accrue for various types of visits. Each type of visit is assigned a certain number of productivity points that is inclusive of 'direct' and 'indirect' patient care activities. Visits are assigned based on patient and business needs. The number of visits performed each week will vary based on individual productivity targets and the productivity points assigned to the visits performed. You will be paid your hourly rate for certain non-visit activities such as orientation. We comply with all minimum wage laws as applicable. Position Details: Our patient ratio on cardiac is 6 patients to 1 nurse. It is a medical/telemetry floor. The main difference in between the Cardiac department and any other medical floor is that we see a few more specialized patients in addition to the typical medical population. For example, we take post op CABG/open heart patients as soon as day 1 following surgery, TAVRs, Watchmen and other post cath-lab intervention patients (stents, AIF, etc.) but they will never have a sheath, they may come to us with a radial band. None of our patients are on vasoactive drips but we do take amio, Cardizem and other cardiac drips and do titrate down to wean but don’t increase the rate on our floor. We don’t do nitro drips. Many of our patients have chest tubes. Acuity is really varied, some patients are simply needing their medications adjusted and must be supervised in the hospital with serial EKGs but otherwise are completely independent, some of our patients are very complex and sick but based on our treatments/interventions are still appropriate for our floor. Qualifications: Education: Graduated from a school of nursing (Associate's Degree, Diploma, or BSN). Licensure: Is personally responsible for obtaining, and maintaining, a current RN license within the hiring state and/or compact licensure in which nursing duties are performed and must meet all state board of nursing requirements. Certifications: Basic Life Support certification through the American Heart Association or successful completion of course within 30 days of hire. Working under the direction of licensed clinical staff and providers, this position prepares patients and procedure rooms, assists with noninvasive cardiac testing and ambulatory monitoring workflows, documents care accurately, and promotes a coordinated, patient-centered experience. The Medical Assistant recognizes and promptly communicates changes in patient condition while performing all duties within established competencies, clinic protocols, and scope of practice. Essential Position Responsibilities Prepare procedure and testing rooms before each patient encounter; clean and disinfect equipment, stock supplies, and verify that required equipment is available and functioning properly. Apply, activate, and remove ambulatory cardiac monitors, including Holter, event, mobile cardiac telemetry, and patch monitors, as ordered and within demonstrated competency. Educate patients and caregivers regarding monitor use, symptom recording, skin care, activity precautions, troubleshooting, device return, and when to contact the clinic. Manage monitor inventory and related workflows, including device assignment, enrollment, return tracking, data routing, and follow-up on overdue or incomplete studies. Support clinic workflow by rooming patients, preparing records and orders, coordinating diagnostic appointments, responding to patient messages, and assisting with referrals or prior authorizations as assigned. Document patient preparation, vital signs, education, device information, procedure support, and other assigned clinical activities accurately and promptly in the electronic health record. Maintain readiness for urgent clinical events by following emergency procedures, confirming access to emergency supplies, and assisting the licensed clinical team during a patient emergency as directed. Follow infection prevention, medication safety, patient identification, privacy, equipment safety, and other regulatory and organizational requirements. Collaborate with electrophysiology providers, nurses, technicians, scheduling staff, and other team members to support timely care and continuity across the patient experience. Perform other clinical and administrative duties within the Medical Assistant’s training, demonstrated competency, and scope of practice as assigned. Qualifications Education: High school diploma or GED required. Completion of an accredited Medical Assistant program preferred. Essential Position Responsibilities Prepare, organize, and maintain the sterile field for cardiovascular, thoracic, vascular, trauma, and related surgical procedures in accordance with established standards, policies, and aseptic technique. Anticipate the needs of the surgeon and surgical team by ensuring appropriate instrumentation, sutures, supplies, graft materials, specialty equipment, and implants are available and functioning throughout the procedure. Maintain strict aseptic technique and actively prevent contamination of the sterile field, including immediate recognition and correction of breaks in technique. Participate in sponge, sharps, and instrument counts in accordance with departmental policy, regulatory requirements, and patient safety standards. Maintain continuous awareness of patient safety, surgical flow, and procedural priorities in a high-acuity cardiovascular operating room and Level II trauma center environment.
