Performs Rapid and Comprehensive Assessments Conducts initial triage and assessment within minutes of patient arrival. Prioritizes care based on acuity using Emergency Severity Index (ESI) or similar triage tools. Completes focused and comprehensive assessments as appropriate to presenting condition . Develops and Implements Emergency Care Plans Identifies immediate patient needs and formulates rapid response plans. Initiates and revises care plans based on evolving patient conditions. Coordinates with interdisciplinary teams to ensure timely interventions. Delivers Timely and Effective Interventions Administers medications, treatments, and procedures per physician orders and protocols. Initiates life-saving interventions including CPR, defibrillation, and airway management. Prepares patients for diagnostic testing, procedures, or transfer to higher levels of care. Evaluates Patient Responses and Adjusts Care Continuously monitors patient status and response to interventions. Reassesses and updates care plans based on clinical changes. Documents all assessments, interventions, and outcomes accurately and promptly. Facilitates Communication and Coordination Communicates critical information to physicians and team members. Provides clear and compassionate updates to patients and families. Participates in shift handoffs, trauma alerts, and interdisciplinary huddles. Demonstrates Clinical Competence in Emergency Nursing Maintains proficiency in emergency procedures and equipment (e.g., IV insertion, EKG interpretation, wound care). Follows protocols for stroke, sepsis, trauma, and cardiac emergencies. Uses clinical judgment to escalate care and activate rapid response or code teams. Assures medication safety by following the “5 Rights” of medication administration Manages Multiple Patients and Prioritizes Care Balances care for multiple patients with varying acuity levels. Delegates tasks appropriately to support staff. Maintains situational awareness and adapts to rapidly changing conditions. Provides Age-Appropriate and Culturally Sensitive Care Adjusts communication and interventions based on patient age and developmental level. Respects cultural beliefs and practices in the delivery of care. Ensures Safety and Compliance Adheres to infection control, safety, and emergency preparedness protocols. Uses personal protective equipment appropriately. Reports unsafe conditions and participates in safety initiatives. Position Summary Vitae Health is seeking a dedicated Nurse Practitioner to provide high-quality, patient-centered medical care to residents in a skilled nursing facility. The NP will assess, diagnose, and manage acute and chronic conditions while collaborating with an interdisciplinary care team to improve patient outcomes and quality of life. Key Responsibilities Perform comprehensive health assessments, diagnose and manage acute and chronic conditions, and develop individualized, evidence-based care plans for residents in a skilled nursing setting Provide ongoing medical management including ordering and interpreting diagnostics, prescribing treatments, conducting medication reviews, and proactively monitoring conditions to prevent complications and reduce hospitalizations Deliver timely clinical interventions in response to changes in patient status, ensuring high-quality, patient-centered care that improves outcomes and supports functional well-being Support value-based care initiatives by focusing on quality outcomes, reducing avoidable hospital readmissions, optimizing cost of care, and utilizing data to guide clinical decision-making and performance improvement Collaborate closely with physicians and interdisciplinary team members to ensure coordinated, efficient care, and participate in care plan meetings and quality improvement efforts Facilitate smooth transitions of care across settings, ensuring continuity, reducing gaps in treatment, and supporting successful admissions and discharges Educate residents and families on diagnoses, treatment plans, medications, and preventative care strategies to promote engagement and long-term health Maintain accurate, timely, and compliant documentation while adhering to all clinical, regulatory, and organizational standards Master’s degree in Nursing from an accredited Nurse Practitioner program Active and unrestricted Advanced Practice Nurse (APN) license in respective state Current DEA registration and prescriptive authority (or eligibility) Board certification as a Nurse Practitioner (e.g., FNP, AGNP) 2 years of clinical experience preferred, ideally in geriatrics, long-term care, or post-acute settings Strong clinical assessment, diagnostic, and treatment planning skills Knowledge of chronic disease management and geriatric care principles Ability to deliver high-quality, value-based care focused on outcomes, cost-efficiency, and reduced hospitalizations Proficiency in evidence-based practice and clinical decision-making Ability to work both independently and collaboratively within an interdisciplinary team Excellent communication and interpersonal skills with residents, families, and staff Strong organizational, time management, and documentation skills Familiarity with electronic health records (EHR) systems Consistent, reliable referral flow so you can focus on patient care, not building a caseload Full malpractice coverage including tail insurance, no out-of-pocket costs Retirement planning support with a 401(k) and company match Ongoing professional growth through CEU opportunities and paid licensure renewals Supportive, clinician-focused leadership team that prioritizes your success and well-being Up to a $3,000 referral bonus, get rewarded for bringing great clinicians to the team Equal Opportunity Statement Vitae Health is an Equal Opportunity Employer and does not discriminate based on race, color, religion, sex, national origin, age, disability, or any other protected status.
