Are you living your life's work? Summary: The CPUP Licensed Practical Nurse assists physicians and performs healthcare services to patients in hospitals, clinics, and other healthcare facilities. Responsibilities: Administers prescribed medications, gives injections and vaccines, reads PPD results. Performs electrocardiograms (EKG) and screening tests. Obtains and tests specimens following standard operating procedures and documents results, taking appropriate action with unexpected results. Prepares specimens for transport as necessary. Gather information from patient and direct to the RN or Provider when necessary. Assists provider with surgical procedures including positioning patients, keeping a sterile field, and wound cleaning and dressing. Prepares paperwork to complete patient visit. Obtains and documents patient's vital signs as needed and reports abnormal results to provider. Cleans, disinfects, sterilizes and checks to make sure all instruments and equipment are in proper working order. Maintains medication storage closets. Participates in orienting new team members. Performs duties in accordance with Penn Medicine and entity values, policies, and procedures Identifies 2-3 participants per month meeting criteria for PMHARC Cohort and helping to recruit for other PMHARC studies Meet regularly (weekly or biweekly) with PMHARC staff members regarding recruitment for PMHARC Cohort and research studies and attend monthly PMHARC/CFAR Research Support Group Proficiency in harm reduction and patient centered care. Ability to perform phlebotomy, wound care, and administer long-acting injectable medications in the medical office and during home visits. Curiosity, humanity and communication skills for complex case management between multiple teams. Work with clinic providers, social workers and staff to identify potential home visit candidates Identify and refer patients to Penn LINK who would benefit from care navigation to facilitate linkage to MOUD care Coordinate and track outreach/home visits including reaching out to patients prior to planned visit. This position plays a crucial role in educating patients, families, and healthcare providers on the hospice benefit. Key Responsibilities: Perform clinical assessments of referred patients to determine hospice eligibility. Collaborate with physicians, case managers, and hospital discharge planners to coordinate timely referrals and admissions. Provide education to patients and families on the hospice philosophy, services, and expectations. Document all assessments, communication, and coordination efforts accurately and timely. Assist in completing necessary clinical and administrative documentation for admission. Serve as a clinical resource to the marketing and business development team. Attend interdisciplinary meetings and participate in community outreach as needed. Qualifications: Licensed Registered Nurse (RN) Minimum 2 years of clinical experience, preferably in hospice, palliative care, or acute care. PURPOSE AND SCOPE: The professional registered nurse Outpatient RN CAP 1 is an entry level designation into the Clinical Advancement Program (CAP). This position is accountable and responsible for the provision and coordination of clinically competent care including assessment, planning, intervention and evaluation for an assigned group of patients. This may include delegation of appropriate tasks to direct patient care staff including but not limited to RNs, LVN/LPNs and Patient Care Technicians. Leveraging Penn Medicine's preferred post-acute care network, the role applies clinical judgment and evidence-based practice to align patient needs, payer requirements, and organizational priorities. The Care Continuum Navigator drives optimal post-acute length-of-stay (LOS) management, reduces avoidable readmissions, and coordinates complex care transitions with a focus on quality outcomes, regulatory compliance, and patient-centered care. In collaboration with physicians, social workers, utilization management nurses, case managers, and post-acute providers, this role addresses barriers to discharge, aligns care plans with clinical criteria and patient goals, and supports successful outcomes across the continuum of care. Accountabilities: Post-Acute Care Coordination & Preferred Network Optimization · Coordinate timely, clinically appropriate transitions to post-acute settings (SNF, LTAC, IRF, HHA, hospice), prioritizing Penn Medicine’s preferred, high-performing network. · Perform concurrent clinical review of referrals to ensure appropriate level of care and medical necessity. · Maintain knowledge of network capabilities, bed availability, and specialty services; align placements with payer requirements and MSSP attribution. Length-of-Stay (LOS) Management · Partner with care teams in both the hospital and post-acute settings to identify and resolve discharge barriers supporting timely patient progression. · Conduct daily discharge readiness assessments and participate in multidisciplinary rounds to align care plans with target discharge dates. · Track and trend LOS data at the unit and patient level, identifying patterns and contributing to system-wide performance improvement initiatives. · Escalate LOS concerns and analyze LOS trends to support performance improvement.
