Approximately 10% of all dialysis patients reside in a SNF and are transported to dialysis three times a week placing a significant burden on patients and adding significant avoidable costs to the health care system. DSD Services teammates will have access to all the capabilities and resources DaVita has to offer, and will work to quickly leverage these to develop the infrastructure and rapidly grow the new service line. Ideal teammates will have experience in dialysis, get energized by a start-up type environment and improving the lives of patients, and have a track record of finding ways to Get Stuff Done (GSD). Teammates must be comfortable working autonomously and have demonstrated the ability to collaborate with cross-functional resources across the organization. What you can expect: In this role, you take responsibility for the complete ongoing care of each patient to ensure their dialysis treatment stays on track. DSD RNs will troubleshoot, and make sound decisions for our patients related to their dialysis treatment and will occasionally partner alongside the SNF staff to coordinate care. Primary work schedule will be 3-4 days a week with some travel to other SNF clinics within your region (depending on business and patient needs). Alternating Saturdays will also be required. The role includes supervising Dialysis Technician who are tasked with delivering tandem care to meet the capacity and efficiency goals of the program. Accepts responsibility for direction of co-workers and delegation of role appropriate tasks in the implementation of care. Provides quality nursing care based on a family centered approach. Provides education for the patient and family/significant other from admission through discharge process. Provides thorough, concise, accurate and legible documentation of the nursing process. Works closely with physicians and other team members addressing patient/family concerns to ensure a smooth transition of care. Administers medications and performs treatments in an accurate safe manner. Ensures clinic processes and workflows comply with FMC policies and procedures. Ensures equipment and supplies are readily available for patient care, reporting malfunctions to proper department. Job Qualifications: Completion of an accredited nursing program, resulting in one of the following: Diploma in Nursing Associate's Degree in Nursing Bachelor's Degree in Nursing Valid license to practice in the State of Ohio. CPR required. Previous nursing experience preferred. Must be able to physically exerting 50 to 100 pounds of force occasionally and/or 25 to 50 pounds of force frequently, and/or 10 to 20 pounds of force constantly to move objects. Physical demand requirements are in excess of those for Medium Work. Position may require frequent patient handling activity.
