Act as a resident advocate and assists residents in developing skills to advocate for themselves Document all appropriate resident information in a timely manner Perform comprehensive nursing assessments on all patients, inclusive of the biopsychosocial and medical history with consideration to age, mental, social and physical status and response. Complete and document ongoing assessment of residents’ normal and abnormal statuses and respond appropriately. Assess and communicate with the registered dietician any special nutritional or dietary requirements. Assess vital signs and report abnormalities to the Medical Staff. Develop resident specific crisis plans designed to assist residents in maintaining safety and reducing need for external intervention. Actively participate in the development of residents’ master treatment plans with the treatment team. Practice within the general policies, philosophy, and objectives of the Department of Nursing and the hospital. Provide nursing care in a therapeutically effective manner demonstrating sound clinical and professional judgment and competency. Participate in community meetings and group therapies when assigned. Assess unit milieu and maintain a safe, therapeutic environment for residents, family members and staff. Complete resident rounds in a timely manner in accordance with requirements and accurately document resident observations. Provide ongoing resident/family education relative to each resident. Accurately enter medication and treatment orders in the EHR and maintain medication administration record (MAR). Accurately and safely administer prescribed medications and treatments to residents using correct methods of administration. Evaluate and document resident response to medication regimen and identify needs to administer PRN medication and document effectively. Assume charge nurse role, as assigned Qualifications: Education/Experience: Graduate of an accredited nursing program Minimum of 2 years’ prior relevant psychiatric experience Massachusetts RN license CPR Certification required prior to employment Must possess knowledge of general and psychiatric nursing processes, therapeutic relationships and processes, age specific growth and development, limit setting, behavior and crisis management Required competencies: Strong organizational and communication skills Attention to detail and ability to prioritize independently Ability to multi-task in a changing and fast paced mental health environment Ability to work in a highly collaborate, team environment Ability to communicate verbally and in writing. Alongside medical providers, case managers, social workers, behavioral health team members and other HIV nurses, you will provide high-quality nursing care to patients consistent with the philosophy of patient centeredness, non-judgement, and team-based care. You will serve in a key clinical role to the care team with primary responsibilities of HIV and other chronic disease management, care coordination, advice/triage needs for the team’s patients, and ensures excellent communication of the care plan across team members. In this role, you will conduct clinical nursing assessments and triage for team patients during routine and urgent medical visits in the outpatient clinic, as well as during outreach visits to shelters, drop-in centers, street venues and patients’ homes. You will also provide care coordination, medication adherence support, health education, and behavioral risk reduction counseling for HIV primary care patients. This role offers a unique opportunity to promote linkage, and retention to HIV primary care, with the aim of supporting people experiencing homelessness and living with HIV to achieve and sustain health and wellness. This position is funded until the end of March 2028 with the possibility for renewal. If this funding is not renewed, this position will end March 31, 2028. Responsibilities: Provides outpatient primary care nursing including medical assessments and triage services for HIV team patients coming in for same-day or scheduled visits; conducts virtual, telephonic, or in-person nurse assessment and triage to assess the severity of the patient’s health concerns using approved protocols and resources to advise appropriately. Documents patient information accurately and in a timely manner in the EHR. Implements the patient-centered plan of care, evaluates outcomes, and regularly communicates the plan of care, patient status, and progress toward goals with the patient and care team members; conducts comprehensive intake assessments for new HIV team patients. Administers vaccines, injectable medications, and provides wound assessment and wound care as indicated; provides medication adherence assessments and counseling. Provides directly observed therapy when indicated with support of outreach team members. Coordinates care with integrated HIV team behavioral health staff. Along with other team members, visits patients at shelters and other outreach sites to provide nursing care and care coordination services and identify barriers to medication adherence and engagement in care. Provides chronic disease management support for HIV as well as other common co-morbid medical conditions such as diabetes and hypertension. Identifies any structural barriers patients face in accessing medical care and adhering to HIV medications, and works with the interdisciplinary team to address those barriers (e.g., housing, transportation, drug treatment, food, clothing, benefits, etc.) Assists medical providers in coordinating follow up related to the provision of medication (such as buprenorphine and naltrexone) for opioid use disorder and other substance use disorders. Responsible for participating in educational, quality assurance and research activities. Additionally, the Registered Nurse acts as a consultant, sharing his/her clinical expertise and knowledge with other health care providers and members of the community. Performs clinical management functions specifically delegated by the Clinical Manager. The nurse participates in organizational planning and decision making which affects patient care and Ambulatory Service operations. The Registered Nurse also participates in related nursing, medical and hospital committee activities. Responsibilities Responsible to the Clinical Manager, meets and maintains competence as defined by the clinical service experience and credential requirements, provides nursing care to patients. Implements and coordinates the steps of the nursing process assessment, diagnosis, goal setting, prescription, intervention and evaluation. Delegates, supervises and coordinates nursing care activities provided by personnel assigned to assist in the provision of nursing care, such as but not limited to Licensed Practical Nurses, Nurses Assistants, Orderlies, and Technicians. Implements the prescribed medical regimen, evaluates the patient response to the regimen and advises the physician of changes in the patient’s condition. Performs extended functions in accordance with written Rhode Island Hospital protocols and policies. Participates in clinic and departmental level collaborative activities such as but not limited to joint case presentations, grand rounds. Collaborates on a continuous basis with the patient, family, Physician, nurses and other health team members to integrate, evaluate and ensure continuity of the plan of care. Identifies with and makes referrals to health team resources within the hospital and the community and serves as a consultant to others. Provides, coordinates and documents teaching for patients and families related to management of the health problem, health promotion and rehabilitation. Seeks out and participates in activities leading to self-growth and professional development. Updates knowledge of professional, medical and technological developments and participates in the incorporation of these into clinical nursing practice. Participates in educational activities commensurate with learning needs and individual ability such as but not limited to conference, seminars and orientation of new staff. Works with facility members of affiliated nursing educational programs to integrate clinical proactive for students within the plan patient care management. Participates in clinic and departmental quality assurance activities such as but not limited to monitoring and evaluation of care, validation of nursing and ambulatory standards, policies and procedures. Participates in related clinical and operational research activities. Assumes charge position responsibilities for the clinic as delegated. Participates with the Ambulatory Clinical Manager in performance appraisals. Participates on committees and in other organizational activities involving professional practice and patient care standards. Minimum Qualifications Licensure as Registered Nurse in the State of Rhode Island by the Rhode Island Board of Nursing or licensure as a Registered Nurse in accordance with the Nurse Licensure Compact agreement of the National Council of State Boards of Nursing. Basic Knowledge Graduate of an accredited School of Nursing, Bachelor’s Degree in Nursing preferred. Must have current licensure to practice as a Registered Nurse in the State of Rhode Island. Demonstrated knowledge and skills necessary to provide care to patients with consideration of aging processes, human development stages and cultural patterns in each step of the care process. Demonstrated competence in administration of chemotherapy and biotherapy with and understanding of COG protocols. At least two years of experience in Pediatric Oncology, including the administration of chemotherapy and biotherapy, within the past five years is required. Active Pediatric Chemotherapy/Biotherapy Provider required. Related experience and credentials where indicated. Work Environment And Physical Requirements Exposed to ambulatory patient care environment which includes exposure to communicable diseases, however adherence to infection control policies and procedures minimizes chance of illness or injury. Independent Action Performs independently within the departments policies and practices.
