We are seeking a self-motivated, highly organized Registered Nurse II to join our Human Genetics Metabolic Disorders team. This role supports patients and families navigating complex diagnoses while serving as a critical member of a multidisciplinary care team. The nurse will work closely with three physician providers, two Advanced Practice Registered Nurses, and two full-time metabolic dietitians to coordinate care and support patients throughout their treatment journey. About the Patient Population The Metabolic Disorders Program primarily serves infants and young children diagnosed with rare inherited metabolic disorders, including: Disorders of protein metabolism, including Phenylketonuria (PKU) Carbohydrate and energy metabolism disorders, including Glycogen Storage Disease Fatty Acid Oxidation Disorders Metabolic /mitochondrial program and 22Q Other rare inherited metabolic conditions Treatment plans may include: Specialized medical formulas Dietary supplements Prescription medications Nutritional management and monitoring Long-term care coordination Position Overview This role combines direct patient care, clinic support, care coordination, and office-based nursing responsibilities. The RN will attend clinic sessions alongside providers and play an essential role in ensuring patients receive comprehensive, coordinated care. Success in this role requires strong organizational skills, attention to detail, independent decision-making, and the ability to manage multiple priorities in a fast-paced outpatient environment. JOB DESCRIPTION SUMMARY The Clinical Director is responsible for the overall direction of hospice clinical services according to the policies and procedures of Continuum Hospice. The Clinical Director supervises the care and services provided by the Interdisciplinary Group and delegates nursing services according to established practice standards. The Clinical Director also establishes, implements and evaluates goals and objectives for hospice services that meet and promote the standards of quality and contribute to the total organization and philosophy. ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES 1. The Clinical Director designates an alternate, in writing, as responsible for fulfilling the duties of the DCS . 2. Supervises all direct and indirect patient services provided by clinical organization personnel. 3. Provides guidance and counseling to coordinators and Clinical Managers to assist them in continually improving all aspects of hospice care services, provided through organization personnel. 4. Assists Clinical Managers in managing clinical teams and planning. 5. Stays informed about changes in the field of nursing and hospice care; shares information with appropriate organization personnel. 23. Promotes hospice referrals in the health care community. 24. In the absence of the Executive Director/Administrator, the Clinical Director becomes the acting Executive Director/Administrator and will be vested with authority to act in behalf of the Executive Director/Administrator. 25. Other duties as assigned by the Executive Director/Administrator. POSITION QUALIFICATIONS 1. Graduate of an accredited school of nursing 2. Registered Nurse, with a License in good standing to practice in the state of Ohio. 3. Either a Bachelor’s degree in nursing with three (3) years of management experience within the last five (5) years in a hospice or related health care organization, at least one (1) year of which was in a supervisory or administrative capacity, OR A Registered Nurse with four (4) years’ experience within the last five (5) years in a hospice, home health agency, primary care clinic or health facility, at least one (1) year of which was in a supervisory or administrative capacity. 4. Once an offer of employment is made, it is contingent upon satisfactory references, as requested, and criminal background checks by regulation. 5. Demonstrated ability to supervise and direct professional and administrative personnel. 6. Ability to market and deal tactfully with customers and the community. 7. Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order. 8.
