What You Will Do Collaborates with the interprofessional team in the delivery of quality patient/resident care under the direction of a licensed nurse. Required Qualifications AHA Basic Life Support for Healthcare Professional (AHA BLS Healthcare) - American Heart Association -REQUIRED Preferred Qualifications H.S. Diploma or Equivalent Other Comparable to certified nursing assistant course. 1 Year recent experience in geriatric setting or recent experience in an acute care hospital. Essential Functions Admissions, Discharge and Transfer Responsibilities May be responsible for some or all of the following duties: Ensures complete room set up for new admission (bedpan, urine container, tissue paper, basin, fresh water, etc.) Meets and greets new patient/resident and family members on admission to unit Orients patient/resident and family to the facility/room. Inventories and documents patient’s personal belongings on inventory sheet upon admission and updates as needed. Accounts for patient’s belongings during transfer and discharge process. Transfers/transports patient to activities, therapies and other departments on time and in a safe manner. As directed by RN, appropriately enters orders related to equipment, supplies and dietary needs; seeks RN clarification when needed to ensure accuracy. D/c’s patient from IDX within expected timeframe of patient leaving unit. Removes all patient’s equipment and supplies upon discharge. Ensures room preparations set-up for next admission. Patient Care Activities Completes and signs appropriate documents accurately and timely. Provides patient care within scope of responsibilities. Provides and documents daily care of patient per unit standards. In monitored units, follows telemetry process and collaborates with central telemetry for initiating/discontinuing monitoring. Identifies and communicates new resident/patient care issues/concerns. Communicates changes in patients/residents condition. Takes rapid action in life threatening situations and immediately notifies the licensed nurse. Communicates or questions requests outside of scope of practice to appropriate personnel. Communicates plans to transfer patients / residents to other departments or discharge to home. Communicates incomplete work requiring follow-up before the end of shift to licensed nurse. Safety Complies with universal precautions and hospital infection control policies. Uses proper body mechanics and available support equipment to ensure patient’s safety and avoid personal injury. Identifies and communicates patients who are at risk for injury to self and others Complies with hospital policy in management of patients in restraints Labels/stores, ensures safety of patient’s belongings (hearing aids, glasses, dentures) Consistently utilizes proper body mechanics and available support equipment to ensure patient’s safety and avoid personal injury Reports personal injury within two hours after incident happened, and completes the ART and state claim forms Follows safety procedures required for equipment use Reports observed hazards and unsafe practice. Job Summary: Coordinates with physicians, staff, and non-Kaiser providers/facilities regarding patient care/population based management for patients in specifically defined geriatric or other specifically defined patient populations in order to plan and implement a comprehensive, multi-disciplinary approach to manage health conditions, utilization of resources and protocols, patient self-care, implementation and evaluation of treatment plan across the care continuum (primary, secondary, tertiary and continued care). In conjunction with physicians, develops treatment plan, monitors care, makes recommendations for alternative levels of care, identifies cost-effective protocols and care paths and develops guidelines for care that may require coordination across systems of multiple providers/services. Complies with other duties as described. Must be able to work collaboratively with the Multidisciplinary team. Essential Responsibilities: Plans, develops, assesses and evaluates care provided to members. In conjunction with primary care and specialist physicians, evaluates and develops baseline medical and psychosocial evaluations and individualized patient care/treatment plans. Recommends alternative levels of care and ensures compliance with federal, state, and local requirements. Develops individualized patient/family education plan focused on self-management; delivers patient/family education specific to a disease state. Encourages member to follow prescribed course of care (e.g., drug therapy, physical therapy). Coordinates care/services with utilization and/or quality reviewers and monitors level and quality of care. Coordinates the interdisciplinary approach to providing continuity of care, including utilization management, transfer coordination, discharge planning, and obtaining all authorizations/approvals/transfers as needed for outside services for patients/families. Makes referrals to appropriate community services and outside providers. Coordinates transmission of clinical and benefit treatment to patients, families and outside agencies. Consults with internal and external physicians, health care providers, discharge planning and outside agencies regarding continued care/treatment, hospitalization or referral to support services or placement. Arranges and monitors follow-up appointments. Coordinates repatriation of patients and monitors their quality of care. Develops and collects data; trends utilization of health care resources. Produces population based reports on outcomes specific to defined patient populations. Participates with healthcare team/providers in actualizing outcomes by planning, evaluating and implementing decisions and strategies to achieve predetermined cost, clinical, quality, utilization and service outcomes. Develops and maintains case management policies and procedures. Identifies and recommends opportunities for cost savings and improving the quality of care across the continuum. Interprets regulations, health plan benefits, policies, and procedures for members, physicians, medical office staff, contract providers, and outside agencies. Acts as liaison for outside agencies, non-plan facilities, and outside providers. Participates in committees, teams or other work projects/duties as assigned.
