ESSENTIAL FUNCTIONS: Registered Nurse Provides professional nursing care as defined in the nurse practice act and under the direction of the Director of Clinical Services, Patient Care Coordinator, or Medical Director. Communicates appropriate status/condition changes to the DCS, PCC or MD, per agency guidelines. Performs job in compliance with Agency policies and procedures and professional and community standards. Participates in coordination of patient care services (hospice and LTC), attends meetings as required, and follows reporting guidelines. Demonstrates proficiency in use of agency chosen software, EMR, iPad documentation and other systems and technology as required. Documentation is complete, legible and clearly reflects care given and is completed by the end of the assigned shift or visit. Demonstrates ability to effectively communicate verbally and in writing/data entry in English. Works in diverse social and economic situations without imposing own values on patient and family. Maintains appropriate boundaries. Receives and processes referral information from Intake. Explains hospice care and Keystone Hospice services and/or facility services and programs to prospective patients/families, physicians, and other appropriate individuals. Consults with the Medical Director, attending physician, and/or mid-level provider (if any) on patient’s eligibility for hospice care. Completes all required admission paperwork (e.g. admission packet) and consents for care with patients/families timely and accurately. Conducts and documents completion of initial assessments accurately and within required time frames. Provides or collaborates with IDT to provide care needed to address patient/family immediate needs, as identified in the initial assessment. Conducts and documents start of/completion of comprehensive assessments accurately and within required time frames for all assigned patients. Coordinates/communicates necessary information with other members of the IDT, consulting physician(s), facility staff, and others as appropriate. Participates in development of the Plan of Care, as appropriate, for each assigned patient. Serves as a resource for all Hospice and/or facility staff to facilitate continuity of patient care and enhance the quality of patient care. Obtains and documents physician orders, as necessary. QUALIFICATIONS/REQUIREMENTS: Hospice & Long-Term Care Registered Nurse EDUCATION & LICENSURE BACKGROUND Registered Nurse with current license to practice in Indiana. Bachelor’s degree in nursing preferred, but not required. Minimum of one (1) year clinical nursing experience; hospice experience preferred, but not required. Demonstrated knowledge of the regulatory requirements for hospice. DEGREE OF TRAVEL Travel between long-term care facilities and/or place of primary residence. Attend bi-weekly IDT meetings, if requested. *Candidates must be fully Vaccinated against Covid 19, or obtain a religious or medical exemption upon hire.

Analyze and interpret patients' histories, symptoms, physical findings, and diagnostic information to develop appropriate diagnoses. Deliver excellent patient care through in-patient and virtual diagnostic and therapeutic recommendations with attention to patient-centric care, safety, cost, and reliably accurate information. Ensure that the activities of the Provider are conducted in a manner that is consistent with overall department expectations and are in compliance with Federal and State regulations, guidelines, and requirements including working knowledge of all health information management issues such as HIPAA. Ability to work efficiently in a fast-paced, autonomous environment within both in-patient clinic and virtual settings. Ability to pursue queue of telehealth patients in a timely fashion to ensure proper patient follow up. Order, perform, or interpret the results of diagnostic tests, as well as responsibly prescribe medications and educate patients on continued treatment and care of acute and chronic conditions. Attest and follow clinical practice guidelines by the Office of Medical Affairs. Attend Mandatory monthly meetings with Supervising physicians and E/M training. Ability to perform responsibilities within standard NP/PA protocols. Proficient in spirometry venous labs draws, centrifuge Dispense medications as directed by the physician Proficient in the use and maintenance of Automated Eternal Defibrillator (AED) Prepares treatment rooms and patients for examination Cleans and sterilizes instruments as appropriate Clerical: Performs administrative tasks, completes appropriate forms, and manages the front desk/check in of patients Answers phones/phone triage and scheduling appointments Responsible for compliance and regulatory reporting Maintain clinic and patient records and telephone encounters Inventories and orders medical supplies, materials, and medications Follow up with center patients who miss appointments Schedule external appointments/referrals and make follow-up phone calls to patients regarding scheduling Ensure completion of all services ordered by the physician such that all reports, consult notes, and follow ups are in the chart for the physician to review Under the direction of the Patient Advocate, periodically assists with engagement/outreach functions Required Knowledge, Skills, & Abilities Required: High school diploma or equivalent Graduation from an accredited Medical Assisting program OR twelve months of direct clinical patient care experience in a healthcare setting OR current Medical Assistant certification Candidates who do not possess Medical Assistant Certification must obtain Certification within 6 months of employment Active and unrevoked certification such as RMA, CCMA, CMA, NCMA, or NCRMA certification is preferred Current BLS certification through American Heart Association valid for at least 90 days after start date Knowledge of Internet software, Spreadsheet software, Word Processing software, and Electronic medical records Ability to communicate effectively and maintain working relationships with people from diverse backgrounds Ability to prioritize needs and plan work accordingly Knowledge of HIPAA confidentiality requirements Work Environment & Physical Requirements This job primarily operates in a medical office environment and is required to interact with patients for the majority of the workday. Must pass a medication administration exam.
