Nurse Practitioner (NP) / Physician Assistant (PA) – Long-Term Care A specialized vascular care group dedicated to serving the long-term care setting is seeking a Nurse Practitioner (NP) or Physician Assistant (PA) to join its growing team. The organization delivers advanced, clinic-level vascular care directly within nursing homes, allowing patients to receive high-quality treatment in a familiar environment. This role involves providing personalized bedside care, reducing the need for patient transport while ensuring comprehensive vascular services—from routine assessments to advanced procedures. Position Overview: The NP/PA will perform on-site clinical assessments and develop appropriate treatment plans for residents in long-term care facilities. This position reports to a Lead Nurse Practitioner and operates within a mobile practice model. Responsible for maintenance of medical records and scanning charts to patient files. Answer phone calls, collect and sort daily incoming and outgoing postal correspondence Ensure reception is well maintained Conduct courtesy callbacks for patient follow-up. Perform opening and closing duties, which include cashing out, end-of-day communication, and securing the building. Follow operating instructions to perform and document daily controls and calibration of equipment; maintain Quality Assurance/Quality Control logs for equipment troubleshoot breakdowns, perform preventive maintenance, and submit repair tickets as needed.
