166199 Job Description Serves as liaison between the patient and facility/physician. Ensures a continuum of quality patient care throughout hospitalization and oversees provisions for patient's discharge. Assesses, plans, oversees and evaluates the appropriateness of care throughout admission and hospitalization of the patient. Job Responsibility Facilitates patient management throughout hospitalization. Performs concurrent utilization management using evidence based medical necessity criteria. Submits data to management regarding case management and/or quality initiatives. Serves as an in-patient liaison - planning, assessing, implementing and evaluating patient in collaboration with the health care team. Conducts chart review for appropriateness of admission and continued length of stay; Facilitates throughput. Conducts clinical reviews and interacts with third party payers to obtain approval of hospital days, pre-certification and post-discharge eligibility in relation to clinical course. Ensures compliance with current state, federal, and third party payer regulations. Documents the case management process in the medical record; completes and documents a care coordination assessment on the patient. Serves as a liaison to patient, family, admitting, primary care physician, health care team, and hospital departments. Completes applicable areas of the patients discharge instruction sheet and the patient transfer sheet; ensures case management documentation is current and accurate. 187312 Job Description Facilitates patient admission, discharge, intra-hospital and inter-facility transfer processes through clinical triage. Acts as the primary liaison between physicians, hospitals and other care providers who seek to refer, schedule or transfer patients into the organization. Provides clinical expertise, close medical communication and quality assessment for the successful referral and transfer of patients to the facility. Job Responsibility Promotes optimum utilization of hospital beds; conducts rounds and meets with staff regarding bed availability and current census. Assigns patients to available beds to avoid delays in patient arrival to units. Communicates with the surgery department, emergency department, nursing units, physician offices and outlying facilities to ensure appropriate placement of patients in a timely manner. Assists with and ensures admission paperwork is complete, and ensures hand-off of communication to accepting nurse. Ensures beds are assigned based on level of care and patient aggregation; addresses and resolves issues arising from increased census and decreased bed availability. Promotes patient, family and physician satisfaction regarding bed assignments; addresses and resolves conflict regarding dissatisfaction with bed assignments. Address any conflicts or issues arising from dissatisfaction with bed assignments. Ensures patient information and data entry in patient’s chart is accurate, including but not limited to insurance information, clinical documentation, transportation requests, and patient demographics.
