This role requires working four 10-hour shifts, ideally Tuesday through Friday, from 8:00 a.m. to 6:00 p.m. Key Responsibilities Of The Hospice Admissions Nurse Provide nursing services to Care Initiatives Hospice patients, through visits that may include telephonic and in-person management of patients, families, caregivers and referral or community sources. Establish positive relationships with community sources with the intent to enhance and expedite patient admissions and coordination of care, medications, equipment, and supplies. Determine immediate needs and initial assessment, share findings with the RN-Case Manager and collaborate care with the IDT. Provide visits to assess ongoing patients’ needs, revise care plans as needed and communicate patient assessments to the team, when not doing admissions.
