Description The (Per Diem) Clinical Nurse is a Registered Professional Nurse who provides safe, competent quality care based on nursing theory and research to a designated group of patients and significant others. Responsibilities Patient Care Delivers patient care by incorporating the tenets of the professional practice model of Relationship Centered Care and through the application of the nursing process based upon theory, research, evidence-based practice, and approved organizational nursing standards. Assesses and evaluates patient care needs and applies critical thinking skills in patient care management. Integrates relevant assessment and intervention skills in the delivery of nursing care. Collaborates with the interdisciplinary healthcare team in the development, implementation, and evaluation of the plan of care. Prioritizes all aspects of patient care including teaching, rounding, coaching, and planning after hospital care and delegating to others as appropriate. Educates patients and caregivers while anticipating needs and readiness to learn, about their plan of care, transition of care, promotion of health, and prevention of disease. Communicates effectively and professionally with patients, family and all members of the Interdisciplinary Patient Care Team. Manages assignments within the Care Delivery Model of Modified Primary Nursing and demonstrates allocation of material resources effectively. Patient Experience Role models the tenets of Mount Sinai Relationship Centered Care (MSHRCC) through facilitation of the following actions including, but not limited to, Admission Welcome, Bedside shift Report, HELP, AM/PM Care, 5 Minute Sit Down, Purposeful Hourly Rounding, Medication Review, Discharge Wrap Up, and MD/RN Unit Collaboration. Supports staff use of evidence-based practices to increase understanding of patients perceptions of care. Conducts Purposeful Hourly Rounding (PHR) on assigned patients addressing the 4 Ps. Quality and Safety Practices safe and efficient patient-centered care, identifying and escalating barriers and opportunities for improvement. Demonstrates clinical skills and knowledge crucial to quality and safety in the patient population served. Implements process improvement strategies based on institutional, evidence-based ensure and procedures. Engages in formal and informal peer and institutional review processes. Collaborates with interdisciplinary teams to create and implement quality improvement projects, evidence-based practice activities and nursing research studies. Contributes to quality and safety practices and required compliance measures. Implements principles of high reliability to identify and avoid high risk behaviors to provide a safe environment for patients. Escalates potential safety hazards or gaps from best practice following institutional chain of command protocol. Utilizes institutional information technology by documenting nursing practice to support quality and performance improvement initiatives. Operations Performs charge nurse duties as assigned. Coordinates with the Clinical Coordinator/Clinical Nurse Manager/Assistant Nurse Manager/Nursing Shift Manager to ensure supplies are appropriately utilized and inventory is sufficient for patient care. Escalates material or staffing resource deficits to charge nurse, Clinical Nurse Manager/Assistant Nurse Manager or Nursing Shift Manager. Demonstrates patient focused and cost effective approaches to patient care in terms of equipment, staff, supplies and all other resources. Documents in an accurate and thorough manner in compliance with hospital, regulatory and legal requirements and standards of care. Professional Development Contributes to the environment of care to support clinical colleagues, patients and their families and members of the healthcare team. Advances clinical competence in nursing practice to progress from novice to expert. Projects a professional image to colleagues and communicates with styles and methods that demonstrate caring, respect, compassion and empathy. (BSN) Associate’s Degree in Nursing (ADN) RN who is matriculated in a BSN Program with a graduation date within the year. Any Associate’s Degree in Nursing (ADN) RN with Nursing experience must be approved by the CNO. Description The Nursing Attendant is an individual who functions in a support role to the Registered Professional Nurse by providing direct patient care and related activities as assigned. Responsibilities Assists the nursing staff in performing selected nursing procedures, treatments, and functional tasks to specific patients including vital signs: Temperature, Pulse, Respirations, Blood Pressure and Pain Rating. Performs procedures associated with activities of daily living, ambulation, mobility, and personal hygiene of the patient. Observes, reports, and records routine and specific patient data, behaviors and responses to treatment on the appropriate flow sheet/EMR. Reports pertinent information and observations regarding patients to the registered nurse Performs procedures related to assuring a safe environment for patients, staff and others. Performs procedures associated with the admission, transfer and discharge of patients. Performs procedures associated with the maintenance of food and fluid intake. Performs procedures associated with excretion of body waste and or loss of fluid. Provides for safe and comfortable lifting, positioning and ambulation of patients in accordance with the principles of good body mechanics. Utilizes mechanical devises as necessary Performs simple applications of heat and cold. Provides care to the body of deceased patients in preparation for removal to the morgue. Provides support to the family/significant others. Secures property. Obtains, assembles and maintains equipment for use during diagnostic and therapeutic procedures. Assists members of the health care team with physical exam and other procedures. Conducts purposeful hourly rounding for assigned patients noting: Pain, Position, Proactive Toileting and Possessions Participates in the patient belonging process, including reporting of valuables to the RN and documenting accordingly. Transports patients by wheelchair, stretcher or bed, and escorts ambulatory patients. Performs errands away from the area of assignment to implement the care of patients Including transporting stat specimens and/or obtaining supplies as necessary. Maintains a clean uncluttered and tidy environment. Maintains equipment and cleans as necessary. Stocks supplies. Works in a safe manner and provides for the safety of patients and other employees. Utilizes aggression control measures in the care of patients who are verbally or physically threatening. Assists the RN with the application of restraints as required. Performs patient observations for Safety, constant/One to One and/or every 20 minutes checks of assigned observation status. Communicates information and basic instructions to patients, families, and visitors Supports the mission, vision, philosophy and goals of the Department of Nursing and The Mount Sinai Health System.
