The APP supports evaluation, education, and longitudinal management for patients with diabetes, thyroid disease, adrenal and pituitary conditions, and metabolic bone disorders. The role emphasizes chronic disease management, patient empowerment, and timely specialty access.Responsibilities • Evaluate and manage adult patients with endocrine conditions, including diabetes, thyroid disease, adrenal and pituitary disorders, and osteoporosis. • Conduct focused assessments, order and interpret diagnostic testing, and develop evidence-based treatment plans. • Support diabetes care including insulin management, CGM interpretation, and patient education. • Monitor and adjust medications and hormone therapies under the direction of the collaborating endocrinologist. • Educate patients and families on disease processes, lifestyle factors, and treatment goals. • Collaborate with physicians, primary care providers, pharmacists, and nutritionists for coordinated endocrine care. • Document care accurately, timely, and in compliance with the EHR standards. • Follow-up on labs, imaging, and referrals to ensure continuity of care. • Participate in quality improvement initiatives, team meetings, and case discussions. • Demonstrate professionalism, collaboration, commitment to team based, evidence-based outpatient endocrinology care.
