Personal Care Assistance: PCA shall assist residents in the dining room during mealtime by distributing meals to residents. PCA shall assist with cleaning the dining room after meals when required. PCA shall distribute facility provided snacks to residents during evening snack time. PCA shall assist residents with showering/bathing as needed. PCA shall assist residents with maintaining a healthy hygiene routine as needed. PCA shall assist residents by laundering residents clothing, linens, and towels as needed. PCA shall assist residents that require limited mobility assistance as needed. PCA shall answer all call bells, and check residents' whereabouts during rounds. PCA shall assist residents with operating the elevator lift. PCA shall assist residents by performing housekeeping and light maintenance tasks as needed. These tasks include but are not limited to: laundering residents clothing, linens, and towels, cleaning resident rooms, making residents’ beds, unclogging toilets, adjusting room temperature, salt areas outside of facility. Cooperate with other resident services when coordinating nursing services to ensure that the resident’s total regimen of care is maintained. Participate in the maintenance and implementation of the Center’s quality assurance program for the nursing service department. Participate in Center surveys or inspections made by authorized government agencies, as may be requested. Periodically review the resident’s written discharge plan. Conducts clinical reviews and interacts with third party payers to obtain approval of hospital days, pre-certification and post-discharge eligibility in relation to clinical course. Ensures compliance with current state, federal, and third party payer regulations. Documents the case management process in the medical record; completes and documents a care coordination assessment on the patient. Serves as a liaison to patient, family, admitting, primary care physician, health care team, and hospital departments. Completes applicable areas of the patients discharge instruction sheet and the patient transfer sheet; ensures case management documentation is current and accurate.
