Responsibilities POSITION SUMMARY Serves as a clinical assistant to the licensed nursing staff by performing nursing procedures, basic patient care tasks in a manner that is reflective the ESC Core Values under the supervision of an RN or LPN. Make two hourly checks on incontinent residents and wash and change as necessary. Encourages self-help measures as appropriate so that each child may reach his/her appropriate level of independence. Check and record urine and bowel movements. Ensure that assigned residents are appropriately prepared for school, clinic, recreation and hospital visits as scheduled and accompany/transport them as assigned. Ensure that bedfast residents are repositioned as directed by NCC but not less than two repositions per resident hourly. Performs restorative and rehabilitative activities as instructed and as directed by Plan of Care. Assists in applying safety restraints to residents when directed; Monitors residents in restraints, release restraints and documents according to policy and procedure Ensures that each resident who can use a call bell has one within reach and remind him/her to use it. Assists co-workers with lifting, positioning and other procedures as necessary. Be aware of facility policy on child abuse and/or neglect and promptly report any instances of abusive or inappropriate behaviors towards any resident to the NCC. Knows and follows The Center policy and procedure with regard to infection control including hand washing and types of isolation. Keep resident care area clean and free of clutter. Report any unsafe conditions immediately to NCC. Properly stores and safeguard residents’ clothing and reports too many or too few clothing and or toys to NCC. Feeds residents or teaches residents to feed themselves as appropriate. Ensure that hands are washed (child and caregivers) and that meal times are pleasant, dignified experiences Qualifications High School Diploma or equivalent required Successful completion of NYS approved Nurse Assistant Certification Program Active CNA Certification on NYS Nurse Aide Registry required Experience Experience working with children. Responsibilities The primary purpose of the Operating Room Technician II position is to be under the direct supervision of the professional nurse. The OR Technician II performs various duties to assist with surgical procedures and related operating room activities. The OR Technician II delivers care as reflected in the Association of Surgical Technologists’ Recommended Standards of Practice in a manner that is cost-effective without compromising quality of care. Potential for exposure to anesthesia waste gases, ethylene oxide, harsh disinfectants, sterilizing agents, high steam pressure agents and radiation. Responsibilities POSITION SUMMARY The Assistant Director of Nursing ensures that standards of professional practice are developed and maintained on a 24-hour basis, 7 days a week to achieve excellence and continuity in resident care on the unit in a manner that is reflective the ESCC Core Values. The Assistant Dir. of Nursing will ensure that nursing care goals are established for each resident and provides guidance for the clinical care team to work toward established goals, ensuring that appropriate nursing care is provided and required documentation is completed. The Assistant Dir. of Nursing will function as an advocate for resident and family, intervene and provide leadership and guidance in crises, and in assuring the resident achieves their highest level of functioning within a least restrictive environment. Responsible for the leadership, direction, supervision, evaluation, hiring and termination of all nursing staff assigned to the neighborhood, the Assistant Dir. of Nursing serves as a support person for members of the staff and acts as a role model to instill in the staff that patient safety, wellbeing and family collaboration are the hallmarks of clinical nursing excellence. The Assistant Director of Nursing will be responsible for working an on-site schedule of a 5 days/week that may include weekends. Provide follow up and corrective action for audits (including medication errors, quality audits, medical record audits, mock survey etc.) Ensure all purchase requisitions and purchase orders are properly prepared, signed and submitted as required. In collaboration with neighborhood staff, ensure adequate and appropriate par levels of supplies and linen. Perform administrative requirements such as completing forms, reports, etc., and submitting to the Director of Nursing as required. Establish a least restrictive environment where restraints are only used after the care plan team agrees that all other alternatives have been exhausted. Responsible for ensuring regulatory compliance for completion of MDS and Care Plans for each resident according to the pre-determined schedule. Assures that all documentation is completed by established deadlines. Ensures coverage for completion of MDS/Care Plans when assigned staff are unavailable. Coordinate and oversee the Care Plan meetings. Responsible for ensuring completion and follow up of occurrence reports, medication error forms, skin care forms and continually work on reducing occurrences and medication errors through root cause analysis, education and quality improvement processes. Rounds on Neighborhoods for the purpose of obtaining clinical information on residents, to support and educate staff, to assist with family issues and endorses complete information to next shift. Promotes 24 x7 accurate, objective clinical and administrative reports. Responsible for review of EMR status board and other variance reports and follow up with staff on outstanding items. Establish a schedule for and coordinate neighborhood meetings. Assure that information is disseminated to all appropriate parties timely. Review complaints and grievances and make oral/written reports to the Director of Nursing and Vice President of Operations. Coordinate investigations as necessary including gathering statements, interviews, review of cameras, photos, disciplinary proceedings and remedial education as appropriate. Coordinate with Talent Management as necessary Collaborates with clinical educators to ensure all required educational opportunities for staff are attended and that mandatory clinical skills competency requirements are completed. Maintains accurate and clear records regarding changes in shift assignment in accordance with rules of the Center and the 1199 bargaining unit contract. Monitor the performance of ANM, ANCC and charge nurses to ensure that they meet facility and regulatory standards in regards to care provided, leadership growth, professionalism, interpersonal skills, team building. Establish a protocol for the interviewing process and orienting new members to the neighborhoods. Facilitate self-staffing in collaboration with neighborhood staff and Staffing Office. Manage budget for neighborhood. Participate in research, evidence-based practice and demonstration of new products/equipment for the Children’s Center. Overview The Observation Unit is specialized and designed for efficient, ongoing medical treatment, assessment, and reassessment of patients before the appropriate decision can be made to either discharge or admit. You will care for a wide variety of patients with chest pain, A-fib, cellulitis, syncope, asthma, and congestive heart failure, trauma, musculoskeletal pain, stroke and TIA, among other diagnosis. This is a fast paced service. Patients have a short length of stay and the unit admits and discharges patients 24/7. Responsibilities The primary purpose of the Staff Nurse on the Observation Unit (H-Blue) is to be accountable for the care activities, safety and goal achievements of assigned patients through assessing, planning and intervening in actual or potential health problems and evaluating patient’s response to the care. Overview About the Role The Oncology RN Clinical Care Coordinator (OCC) works collaboratively with an oncologist in delivering care to their assigned patient population. This is an outpatient position and requires oncology clinical experience and sound nursing knowledge related to the care of patients undergoing cancer treatment to assist with the education, the coordination of care, and ongoing assessment of the patient's response to treatment. The OCCC assesses patients for symptoms, clinical and psychological needs, treatment-related concerns, and barriers to care.
