Job Responsibility Manages and leads team members, operations and patient care activities within a patient care unit during assigned shift. Collaborates with team members and nursing management in the development of methods, techniques and analytical approach to patient care. Communicates, builds consensus, and guides team members toward achieving department objectives. Promotes and maintains high quality patient and family centered care in a healing environment. Educates team members in new and existing department procedures and systems (information systems, fiscal responsibility, etc.). Provides input on department budget allocations to nursing management. Ensures adherence to the established care delivery model, using knowledge of scope and standards of practice. Faciliates continual patient care unit compliance with the JC, CMS, New York State, DOH, Public Health Law, HIPPA regulations and System, hospital and department standards. Demonstrates core clinical competencies for patient population of oversight in order to supervise and provide direct patient care. Maintains current knowledge of scope and standards of practice, healthcare legislation, and population health. Interprets policies, procedures and technical methods for team members. Identifies opportunities and makes recommendations that improve patient care delivery and team effectiveness using evidence-based strategies and solutions. Performs related duties as required. Job Qualification Bachelor’s Degree in Nursing (BSN) required. Current license to practice as a Registered Professional Nurse in New York State, required. Current License to practice as a Nurse Practitioner in New York State required, plus specialized certifications as needed. OR Master of Science required. Current License to practice as a Physician Assistant in New York State required, plus specialized certifications as needed. Description A nurse practitioner is certified under section 6910 of the New York State Education Law and licensed as a registered nurse. A nurse practitioner provides acute/primary health care in accordance with written policies approved by nursing and medical administration. Responsibilities 1. Develops and implements a collaborative practice agreement in the specific clinical area and performs a periodic review of the collaborative agreement. 2. Assesses the health status of patients/families by means of comprehensive health history and physical examination. 3. Initiates selected therapeutic and diagnostic procedures, consultations and laboratory examination, prescribes and alters medication regimens, interprets findings and alters plan of care according to collaborative practice agreement and protocols in designated area. 4. Provides teaching and counseling in the areas of health promotion, maintenance, disease prevention, and management of acute/chronic illnesses. 5. Documents findings and plan of care in the patient's medical record. 6. Consults with physician as established in the collaborative practice agreement. 7. Initiates referrals to specialty services and fosters continuity of care. 8. Participates in team meetings and conferences to enhance an interdisciplinary approach to acute/primary health care delivery. 9. Performs a patient record review with the collaborating physician in a timely fashion but not less than every three months Professional Development: 1.
