Complete monthly summary assigned for shift Complete skin assessment assigned for shift Ensure all CNA’s documentation on ADLs and meal intake is completed before end of shift. Ensure all shower sheets are turned in to you and reviewed before end of shift, turn in to Unit manager or DON (under her office door). Complete all MDS charting, Skilled charting, changes in condition and/or new orders or treatments received for specific resident in their charts. Please document schedule or cancelled doctors’ appointments in residents’ chart. (inform transportation of changes) For OOP residents please fill out appropriate form, and head to toe assessment completed. Complete incident report if any occur during shift (write incident on 24 hour report, and submit paperwork to unit manager or slide under DON’s door). Complete lab inquiries and noted on lab log for any new orders or any labs received (log them in).

Position Details Monday –Friday, Full Time Schedule, 40 hours per week Diagnose and treat acute and chronic conditions, illnesses, and injuries EPIC EMR documentation system Educate patients and their families on health maintenance and prevention Comprehensive benefit package including health, life, dental, vision CME, Malpractice and 401k Requriments Graduate of accredited Nurse Practitioner program NP Certification and Texas license required Nurse Practitioner practice experience a plus but not required The Nurse Practitioner (“NP”) Must be a graduate of an accredited Advanced Nursing Program and have a current RN license for the State of Texas. Certified Advanced Nurse Practitioner in the State of Texas. The NP will report to the supervising physician and/or the Medical Director and will be responsible for meeting CME requirements to maintain certification and licensure.
