Description The Nurse Navigator in Infusion serves as a clinical expert coordinating and administering patient care treatment regimens, research components and program outreach for patients diagnosed with cancer. The Navigator will assist patients and their families in managing through the vast array of services and programs offered by the hospital and the community to meet their health and wellness needs. The Nurse Navigator will provide personalized clinical and high touch care, advocating for the integration of whole person health of body, mind, and spirit. Medical City Fort Worth Transplant Coordinator Full Time, Exempt Medical City Fort Worth is a 350+ bed full-service Magnet Designated hospital. It is located in the heart of the medical district. Medical City Fort Worth serves as a tertiary referral center for Tarrant County and many counties within a 90 mile radius. We offer comprehensive diagnostic and treatment services. Our specialties include cardiac care, neurosciences and oncology. We have surgical services, orthopedics, kidney transplants and emergency care. We offer three ER locations, including two off campus ER located in Burleson and White Settlement. Medical City Fort Worth is a designated comprehensive stroke center. We are a Joint Commission chest pain center. We are a part of the Medical City network of hospitals. Patients are ensured access to medical experts and advanced healthcare innovations across the network. The Solid Organ Transplant Coordinator is a registered nurse who plans, coordinates and implements the care of the solid organ transplant patient and living donor as applicable in both the in-patient and out-patient settings in accordance with the nursing process . The scope of the role includes acting as liaison with the organ procurement organization and the OPTN, assesses human, financial and material resources of patients and families; coordinates information flow among medical, ancillary, nursing, financial and insurance company staff; represents the transplant program through involvement in hospital and community activities; participates in the development and maintenance of the transplant statistical database . Employee will recognize the population specific needs of the transplant patient and apply the appropriate knowledge , skills and behaviors to meet those needs when performing patient activities.
