A Brief Overview The Home Care Intake Coordinator is responsible for obtaining the referrals necessary for the initiation of Home Care services and their successful reimbursement. Interprets Home Care policies to patients/families, hospital staff, and referral sources. Responds to telephone inquiries regarding information about home care services, makes appropriate calls to obtain referral and/or provides information for alternative community resources. Incorporates the clinical history, physiological and psychosocial of patients into the generation of a referral to insure appropriate discipline referral. Participates in in-services, continuing education, committees and interdepartmental meetings as assigned. Maintains and respects staff and patient confidentiality. Demonstrates compliance with the Hospital and Home Care dress code policy. Observes hospital and agency policies concerning Fire and Safety regulations, Infection Control Standards, and other rules governing conduct while at work.
