Proactively advocate for residents at the clinic to ensure all needs are met and communicate challenges to the facility as needed. Represent Briarcliff Manor Center at all times, adhering to facility policies and procedures, as well as abuse laws, both on and off facility property. Additional clinic visits may be required depending on facility needs. Advise the Clinic Coordinator between appointments if assigned to unit staff duty. Description A nurse practitioner is certified under section 6910 of the New York State Education Law and licensed as a registered nurse. A nurse practitioner provides acute/primary health care in accordance with written policies approved by nursing and medical administration. Responsibilities 1. Develops and implements a collaborative practice agreement in the specific clinical area and performs a periodic review of the collaborative agreement. 2. Assesses the health status of patients/families by means of comprehensive health history and physical examination. 3. Responsibilities for the (LPN) Licensed Practical Nurse • Record a patient’s medical history accurately • Take and record measurements of blood pressure, temperature, heart rate etc. • Observe patients under treatment to identify progress, side-effects of medications etc. • Monitor patients’ condition including fluid intake and output and compose patient charts • Assist ailing patients in daily necessary activities such as eating, bathing etc. • Administer injections, prescribed medications, enemas etc., clean and dress wounds and assist with other basic medical care tasks • Collect samples for lab testing as assigned • Provide emotional and psychological support when needed • Communicate with patients’ family or friends to provide advice, comfort and release instructions

Benefits to working at the Phoenix Center: Competitive salary Medical, dental, and vision benefits 401K retirement plan Paid time off Differential pay Referral bonus Service awards Transportation service Responsibilities as a Registered Nurse: Be able to handle complexity of a Pediatrics unit Ability to care for patients with NGT, GT, Tracheostomy, wound care, and temperature instability Be able to care for patients on a ventilator machine Play an active role in direct resident assessment and care Supervise and coordinate nursing personnel in providing direct resident care in adherence to State and Federal regulations Dispense medication and perform treatments in accordance with Phoenix Center policies and procedures Formulate individualized nursing care plans utilizing the nursing process Assess each resident daily and implement a change in the course of action as needed Assist the physician on rounds and ensure that pertinent information is communicated to and from him/her and orders are taken correctly Participate in the training and supervision of nursing personnel Assists with implementation of facility’s Quality Improvement Program Assume responsibility for the identification of potential safety hazards and for the identification of non-compliance with facility guidelines Maintain accurate resident care records and documents Attend and participate in the resident care meetings, staff meetings, and in-services Take an active role in teaching and guidance to residents, families, and staff Assure that each resident’s attending physician and family, or responsible party are promptly notified of any significant change in the resident’s health condition Effectively utilize the general principles of leadership and supervision Manage conflict through effective problem-solving and communication skills Act calmly and efficiently in handling emergency situations Requirements for a Registered Nurse: Completion of an accredited school of practical nursing Current license as a Registered Nurse in the State of New Jersey This position is a Union position.
