Inside Our Family Birth Center The Grosse Pointe Hospital Cotton Family Birth Center/Women’s Health Unit is comprised of 11 LDRP rooms, a Triage room, and a Natural Birth Suite. This Natural Birth Suite is used often by our Nurse Midwives and patients who want to enjoy a low intervention birth experience. In addition, adjacent to the FBC is a 9-bed unit that cares for gynecologic and medical patients. We also care for readmitted post-partum patients, newborns less than 28 days old, and low risk ante-partum patients. Scope of work The Clinical Nurse Manager is an experienced leader responsible for managing assigned areas and services and supervising assigned patient care staff to promote the creation of a center of excellence for staff recruitment and retention and clinical and business outcomes. The Nurse Manager is responsible for ensuring safe and effective delivery of patient care and clinical outcomes, compliance with regulatory, administrative and clinical standards, and policies and procedures. The Nurse Manager serves as a chief retention and safety officer. He/she ensures that safety and quality standards are upheld by effective use of data and technology to make decisions and proactively mitigates unnecessary risks for his/her area of responsibility. The Nurse Manager creates and ensures a desirable, safe work environment that enables the organization to meet or exceed its mission and goals. He/she models the way to foster communication and collaboration with physicians, staff, patients and other departments. Role specific competencies, responsibilities, roles and functions are based on the Professional Practice Model. This position reports to the Director of Nursing or above. AREAS OF RESPONSIBILITY: Under general direction, maintains 24-hour responsibility and accountability for providing leadership and management of a designated patient care unit which promotes quality nursing care, cost effective use of resources and professional growth for nursing staff. SUMMARY: The Registered Nurse Case Manager (RNCM) of the PACE Southeast Michigan (PACE SEMI) utilizes a systematic approach to nursing practice which incorporates all aspects of the nursing process including, assessment, planning, implementation and evaluation of frail elders with complex needs. The RN demonstrates a direct relationship between nursing interventions and participant outcomes, demonstrates clinical competence and engages in effective patient teaching in areas of prevention as well as treatment. The focus of care is one that enhances functional capacity, encouraging autonomy in all aspects of care, and assures coordination of all nursing care. SPECIFIC DUTIES AND FUNCTIONS: The RNCM assesses participants’ needs and plans for appropriate nursing care upon the Initial Intake Assessment as well as upon routine Re-Evaluation Assessments. The RNCM works and collaborates with the participant and the family, as well as all members of the multidisciplinary Team in developing the participant’s plan of care. The RNCM maximizes the participant’s functional capacity by encouraging autonomy in all aspects of care. The RNCM teaches, supervises and counsels the participant, or caregiver regarding nursing care needs and other related problems. The RN utilizes adult learning principles when planning for and implementing educational information to the participants, caregivers or family members. The RNCM initiates preventative and rehabilitative procedures or programs as appropriate for the participants’ care and safety. The RNCM administers medications and treatments, as ordered by the physician/NP, and monitors the participant’s response.
