Job Summary In the ICU of a hospital, patients are often in critical condition and need constant monitoring and care. ICU nurses review data from special equipment to track a patient’s heart rate, blood pressure, breathing and other vitals. They also carry out more traditional nursing duties such as administering medications and communicating with patients regarding their condition. Essential Functions Performs a thorough and timely assessment of patient’s condition including physical, psychological, social and rehabilitative aspects. Identifies problems/nursing needs including discharge needs. Intervenes appropriately to address identified problems and needs. Evaluates effectiveness of care and revises plan if necessary. Transcribes, verifies and carries out physician orders in an accurate and timely manner Manages time efficiently/sets priorities appropriately. Initiates and effectively maintains IV therapy. The LPN will support individuals living in an Intermediate Care Facility (ICF) Group Home by promoting health, safety, and wellness. Key Responsibilities Supervise and assist direct support staff with personal care and daily living activities. Administer medications according to established procedures. Monitor infection control practices and maintain first aid supplies. Document medical findings and report significant changes to the RN Supervisor. Communicate with healthcare providers and family members regarding medical concerns. Provide treatments including wound care, G-tube feedings, blood glucose monitoring, urinary drainage maintenance, and range-of-motion therapy as ordered. Monitor vital signs, weights, and other health indicators. Ensure compliance with federal and state regulations. Attend medical appointments and required training. Perform other duties as assigned. Qualifications Current unrestricted New York State Licensed Practical Nurse (LPN) license. AHA or equivalent CPR certification. Experience with individuals with intellectual and developmental disabilities preferred but not required. Excellent written and verbal communication skills.
