Develop, monitor, and update individualized health goals. Ensure infection control standards and universal precautions are followed. Maintain necessary medical and first aid supplies. Provide on-call phone consultation as needed. Partner with case management teams to coordinate medical and behavioral health services. Provide health education to individuals, families, and direct support professionals. Coordinate hospital discharges and community reintegration services. Arrange follow-up care with primary care providers, specialists, and community resources. Serve as a clinical liaison between providers, families, and program staff. Participate in interdisciplinary treatment planning and case conferences. Train and mentor Direct Support Professionals (DSPs) on health-related procedures and emergency response. Maintain accurate and timely clinical documentation. Qualifications Current unrestricted New York State Registered Nurse (RN) license. Minimum of two years of nursing experience. Experience working with individuals with IDD, behavioral health, crisis care, or community health is preferred. Knowledge of OPWDD, OMH, and DOH regulations is preferred. Strong assessment, crisis intervention, care coordination, communication, and documentation skills. Ability to work independently and collaboratively within a multidisciplinary team. Ensure a proper orientation and training program for all clinical and technical staff. Ensure that continuing in-services are provided to all clinical and technical staff. Ensures provision of dialysis and related services such as hemoperfusion, apheresis, continuous renal replacement therapies, peritoneal dialysis, and UF complete if contracted to do so. If appropriately licensed, performs treatments as necessary to assist in times of high census, staffing shortages, and/or to assist with managing staffing costs as appropriate. Confer routinely with Technical Program Manager and Technical Operations Manager to ensure preventative maintenance program for routine machine maintenance, to ensure the timely repair of disabled machines, to coordinate infection control measures, routine culturing, water testing, logging of results, transportation of equipment and supplies and communicate technical information to the staff. Ensure all acute treatments are performed in contracted hospitals according to the established policies and procedures and in compliance with all applicable regulatory requirements. Ensure efficient utilization of supplies and equipment. Assist with various projects as assigned. Other duties as assigned.
