Illustrative Duties Graduate Nurse (T&RC) Prepares or assists with preparation of nursing care plans consistent with individuals’ comprehensive treatment plans and medical needs. Carries out treatment procedures, administers medications, and monitors individuals’ conditions. Records notes and maintains individuals’ medical records and nursing care plans, including nursing care, medications, and treatments, reactions to care and treatment, intake and output of liquids and solids, and vital signs. Overview Provides mental health assessment of and treatment to patients with psychiatric symptoms, cognitive deficits, and/or behavioral problems. Provides support and education to VNS Health staff and caregivers to enable them to better care for patients with psychiatric, cognitive and/or behavioral problems. Works under moderate supervision. • Conducts psychosocial assessments and living conditions of patients and develops treatment recommendations. • Travels to patients’ homes and/or other facilities with varying environments (e.g., elevated buildings, walk-ups, care facilities, single/multiple family homes, presence of pets, etc.) using approved transportation options to deliver direct care to the patient. • Transports and utilizes required medical equipment and supplies using VNS Health designated/supplied carrying case weighing approximately 25 – 30 lbs. to and from patient homes/care facilities, VNS Health offices and other locations. • Assists patients with activities of daily living, which may require positioning, moving, transferring and lifting patients of varying weights and physical conditions, with and without assistance and as appropriate, from family members and/or paraprofessionals. • Provides nursing care in accordance with Agency policies, practices, procedures and Standards of Nursing Practice, which may require standing, stooping, sitting, crouching, bending and stretching to deliver patient care. • Provides any of the following services to patients: advocacy, psycho-education, cognitive/behavioral interventions, monitoring of mental status, emotional support, brief psychotherapy, family based interventions. • Provides crisis intervention as necessary. • Consults with Program Coordinator as necessary to develop psychiatric evaluation and treatment plan. • Consults with referrer on assessment, recommendations, and update on progress of treatment. • Provides education, consultation and support to VNS Health staff. • Assist the health care provider in implementing the plan of care. • Follows a treatment plan, revising and updating as necessary. • Makes appropriate referrals for mental health services. • Participates in interdisciplinary team meetings, case conferences and clinical supervision. • Attends in-service education programs. • Assumes responsibility for continued professional growth. • Maintains productivity sufficient to meet VNS Health Home Care standards. • Maintains records in accordance with AMH and VNS Health record keeping standard and performs established workflow procedures. • Participates in special projects and performs other duties as assigned. Description The Clinical Nurse is a Registered Professional Nurse who provides safe, competent quality care based on nursing theory and research to a designated group of patients and significant others. Responsibilities Patient Care 1. Illustrative Duties Nursing Assistant 1 Under Direct Clinical Supervision, Performs The Following Duties Changes simple dressings as directed, gives enemas, douches, and irrigations as directed. Applies abdominal “T” binders, hot water bottles, aqua “K” pads, ice bags, and coolers, and hot and cold compresses to patients as directed. Assists patients in collecting urine and stool specimens. Observes and records patients’ quantitative food and fluid intake and waste output. Takes temperature, pulse rate, blood pressure and respiration rate. Gives therapeutic baths such as sitz and fever sponge baths. Measures and records height and weight. Inspects functioning oxygen therapy equipment including tents, masks, and cannulae to ensure appropriate functioning. Recognizes and reports abnormal signs and symptoms of common diseases and conditions to clinical staff. Applies splints, braces, slings and other self-help devices. Operates a variety of specialized patient monitoring and treatment devices. Attends to patients’ hygiene and daily living needs in order to maintain an atmosphere conducive to patient comfort and treatment. Examples include bathing and dressing patients, brushing teeth, and trimming nails. Collects and carries patients’ laundry and dry cleaning and delivers objects to appropriate locations in the health care facility. Receives patients’ belongings and transports objects to safe storage, keeping appropriate records. Feeds patients requiring assistance. As directed by clinical staff, moves, positions, lifts and transports patients using a variety of lifts and other aids in order to make patients comfortable, prepare patients for treatment, deliver patients to treatment areas, and to prevent contractures. Brings bedpans and urinals to patients requiring assistance and attends to incontinent patients. Talks and listens to patients and responds by encouraging patients to cooperate and participate in treatment activities. May transport patients and supplies on occasion. May perform a variety of limited housekeeping and cleaning duties related to direct patient care in order to ensure that treatment and patient areas are clean, sanitary, orderly, and properly supplied. Nursing Assistant 2 Under direction of higher-level treatment staff, performs duties similar to those of a Nursing Assistant 1, but with greater proficiency. Positions may assist with training new staff in assigned treatment areas. Nursing Assistant (Certified) Under direction of higher-level treatment staff, performs duties similar to those of a Nursing Assistant 2, but also participates in required quarterly in-service education to maintain certification and meet all necessary requirements to be re-certified every two years. Communication Nursing Assistants have frequent face-to-face communication with patients and visitors for the purposes of conveying treatment procedure and visitor information.
