Your Role on the Team The position of Nurse Practitioner II in the Department of Medicine, Division of Hospital Medicine & Palliative Medicine, requires a highly competent, independent yet team-oriented individual who will be immersed in Hospital Medicine. Under the supervision of Anuj Ohri, MD and guided by the applicable policies and procedures, the Nurse Practitioner will participate in the comprehensive care of patients in the ambulatory or inpatient settings and provide multi-disciplinary care coordination, treatment, education, monitoring, and communication with patients and referring physicians. A Nurse Practitioner is an Advanced Practice Provider (APP) who possesses the advanced education, knowledge, clinical skills and experience necessary to provide advanced nursing care in their area of specialization. This Position May Perform Functions as appropriate to their “scope of practice” (In general, the “scope of practice” includes the management of chronic stable medical conditions, routine care, acute or critical care, and medical and surgical procedures per their approved standardized procedure). Familiarity with state statute for their licenses is necessary. Approved privileges are unique to each person and are unlikely to include ALL privileges The NP professional may provide patient care in age groups based on educational preparation and at all Health licensed settings and affiliates. The NP has responsibility for the standards of care for their specified patient population and evidences self-direction and accountability both to quality of care and in the development of their role within the all Health locations and affiliates. Advanced Practice Practitioners (APP) are required to be credentialed to practice and be privileged through Medical Staff to function in their clinical role. Job Summary Provides quality patient care to critically ill patients, through the nursing process of Assessment, Planning, Intervention, and Evaluation. Serves as a patient advocate at all times. Supervises the nursing care provided to each patient by other caregivers. Develops the plan for nursing care as a result of coordination with the patient, family and staff of other disciplines as necessary. Applies critical thinking in order to promote positive clinical outcomes. Ensures patient education. Participates in discharge planning. Demonstrates accurate knowledge of critical Lab values and their normal ranges. Reports and documents appropriately. Demonstrates ability to verify correct lead telemetry placement. Validates telemetry strips every 4 hours and PRN. Documents appropriately. Participates in assessing and meeting patient and family education needs. Provides complete patient report to oncoming RN, ensures documentation is complete, physician orders are signed off, and patient rooms are in order. SUMMARY Under the general supervision of the Director of Nursing, the Registered Nurse performs a variety of resident care functions and closely observes residents for changes in medical status. This position also uses independent judgment and discretion in maintaining the delivery of quality resident care by assuring that the nursing and medical care plan is followed, and makes supervisory decisions. In the absence of the Director of Nursing, this position assumes full responsibility for the resident care. ESSENTIAL RESPONSIBILITIES AND JOB FUNCTIONS Nursing Care : Administers nursing care as indicated by the nursing care plan and in compliance with all Regulatory Agencies. Administration of Resident Care : Demonstrates ability to provide appropriate and quality nursing care to residents, including but not limited to: (1) Reviews all resident histories to determine course of illness and all co morbidities for integration into the care plan. (2) Demonstrating the ability to assign meaning to resident symptoms and initiating appropriate action. (3) Ensuring that problems/concerns regarding activities in the facility/department are communicated appropriately. (4) Effectively assessing, planning, implementing, and evaluating nursing care for residents. Resident Medications: Demonstrates ability to provide appropriate nursing medication management including, but not limited to: (1) Administering medications as ordered by physician using age specific monitors. (2) Ensuring medications are ordered, received and transcribed on a routine basis and in a timely manner. (3) Appropriately transcribing, administering and documenting medications according to policy and procedures and age specific considerations. (4) Monitoring and removing any discontinued or expired medications from medication carts. (5) Maintaining a current, accurate listing of all resident medications. (8) Completing appropriate forms and returning unused medication to the pharmacy in a timely manner.
