Newport News, Virginia Labor Pool Flexible Hours Available Works in collaboration with RHS Healthcare teams in assisting with transition of critical and non-critical patients 24/7 to Riverside Health System's Acute and Emergency facilities and services. Manages all inbound and outbound calls for critical transfers through the Riverside Transfer Center. Performs critical and non-critical patient symptom triage through the Riverside Nurse Triage line. Consistently coordinates patient symptom based triage, facilitates direct inpatient and emergency department admits, and consults. Coordinates and arranges patient transport based on clinical assessment to ensure efficient patient throughput for both critical and non-critical patients. Coordinates and facilitates appropriate patient bed placement based on collaboration with physician and critical clinical assessment for Riverside Health System Acute Care facilities. What you will do Demonstrates ability to use clinical knowledge around clinical service lines, disease entities, and epidemiological issues that could affect transport, care, and placement while demonstrating a sense of urgency to facilitate patient throughput to assure patients receive consistent, competent, and ethical care. Adheres to critical and non-critical protocols, algorithms, policies, and procedures. Accurately completes all required clinical and quality documentation consistently per RHS standards and regulatory requirements. Performs critical and non-critical symptom-based triage through the Riverside Nurse line providing patient disposition from 911 to home care. Demonstrates understanding of diverse patient care populations and ability to modify education method to ensure patient/family comprehension. Coordinates and communicates with transfer team members, medical service transport coordinator, physician, bed management, and Transfer Center medical director, throughout the course of transfer or direct admission to ensure positive patient outcomes. Promotes a network of other Riverside hospitals sending and receiving facilities to facilitate transfers efficiently and timely. Communicates detailed information about patient status and demographic information when arranging critical and non-critical transport. LPN - Clinical Supervisor

Summary GENERAL SUMMARY The LPN Clinical Supervisor reports to the designated manager and is responsible for supervising, coordinating and directing the work flow and functions of designated service personnel to facilitate safe and efficient work flow and practices. ESSENTIAL DUTIES AND RESPONSIBILITIES Assists in the selection, training, orientation and competency validation of designated personnel. Supervises, plans, develops and organizes services and activities to support medical practices. Works in close cooperation with and under the clinical guidance of the practice physician(s). Performs a wide variety of patient care activities and accommodative services to support the medical practice. Demonstrates excellence in nursing practice characterized by quality patient care and participation in formal and informal patient and staff education. Performs other duties as assigned. LICENSES AND/OR CERTIFICATIONS Current Virginia state license as a Practical Nurse (LPN) or Practical Nurse holding a valid Compact State license. CPR Training required and/or must be obtained within 45 days of hire. MINIMUM EDUCATION AND EXPERIENCE REQUIREMENTS Technical and professional knowledge and skills for clinical operations of the service area usually acquired through 3 - 5 years’ progressive Clinical/Supervisory experience in a practice setting. Professional knowledge of nursing theory and practice. Pediatric experience preferred. Must provide record of a completed diploma (or equivalent) or academic transcript for those areas performing complexity testing. WORKING CONDITIONS Clinical patient care environment with minimal exposure to hazards such as excess noise, dust, or extremes in temperature. Frequent exposure to communicable diseases, toxic substances, medicinal preparations, body fluids and other conditions common to the healthcare environment.
