We are seeking a Field Nurse Case Manager to cover Virginia Beach and surrounding communities to include but not limited to Norfolk, Newport News, Chesapeake, Hampton. A Field Case Manager role is a field / home-based position with travel, up to 2 hours one-way (4 hours round trip), to doctors’ offices, hospitals, and various other locations. This individual is responsible for the medical case management of work-related injuries, which includes assessment, planning, coordination, implementation, and evaluation of injured/disabled individuals. The Case Manager works with insurance carriers, medical care providers, attorneys, employers, and employees, and closely monitors the progress of the injured worker and reports results back to the employer and insurance carriers. This is a wage position limited to working no more than 1,500 hours per agency during the 12 month period of May 1 – April 30, regardless of the hire date. The wage employment period resets on May 1 of the following year. All wage employees are limited to working no more than 29 hours per week on average over the 12-month period. This wage position offers no state benefits. Minimum Qualifications Graduate of an accredited school of nursing Must possess and maintain a valid unrestricted Virginia LPN license issued by the board of nursing or a multi-state compact license (if you are a recent graduate of a nursing program in VA and not licensed, the NCLEX-PN exam must be scheduled at the time of application) Working knowledge of principles, practices, and techniques of practical or general nursing care. Demonstrated ability to screen complaints of physical ailments and refer necessary cases to other medical professionals. Ability to maintain appropriate documentation, provide first aid, and carry out providers' orders in applying treatment. Performs administrative support responsibilities. As directed and in accordance with documented competencies, provides administrative support to licensed healthcare professionals. Administrative support services include but are not limited to draft letters and/or assist with completion of forms, facilitate scheduling, pend medication refills, assist with coverage of in basket messaging, assist with clinic/chart prep. Demonstrates teamwork in the delivery of patient care. Recognizes the importance of time sensitive issues. Assumes responsibility for professional development of self and others according to departmental policy. In addition to the above job responsibilities, other duties may be assigned. MINIMUM REQUIREMENTS FOR LICENSED PRACTICAL NURSE: Education: Graduated from an accredited Licensed Practical Nurse Program Experience: None Required Licensure: Licensed to practice as a Practical Nurse (LPN) in the Commonwealth of Virginia prior to onboarding. American Heart Association (AHA) Healthcare Provider BLS certification required Physical Demands: Job requires standing for prolonged periods, frequently traveling, and bending/stooping. Proficient communicative, auditory, and visual skills; attention to detail and ability to write legibly; ability to lift/push/pull 20-50lbs. Current Medical Assistant Certification through a recognized national professional association such as the AAMA (American Association of Medical Assistants), the AMT (American Medical Technologists), NAHP (The National Association of Healthcare Professions), the ARMA (American Registry of Medical Assistants), or the NHA (National Healthcare Association).
