Change bed linens, diapers and clothing as often as necessary. • Do not permit residents to smoke in bed. • Assure that the nurse’s call system is attached to the bed and within easy reach at all times when residents are bedfast. • Answer residents’ call lights promptly. • Check for food in residents’ room. Ensure that food is kept in a sealed container. • Report all complaints to the Charge Nurse. • Report any resident abuse immediately (ie., harsh/abusive language, unnecessary roughness, etc.) • Report any medication found in the resident’s room. Take to the Charge Nurse immediately. • Report all hazardous conditions and faulty equipment to the Charge Nurse immediately. • Report any bruise, skin tears, incidents or accidents to the Charge Nurse immediately. • Attend in-service educational programs, on the job training programs and meetings as directed. • Other related duties as may become necessary or as directed by the Charge Nurse, RN Supervisor, Director of Nurses, or Administrator. Shampoo hair on shower days. • Clip and trim fingernails. • Shave male residents daily or as needed. • Keep hair under arms and face on female residents clean-shaven as necessary or instructed. Trim hair in nose and ears as needed. • Observe and report the presence of reddened areas on bony prominences. • Keep drainage bag off the floor when resident is in bed or in wheelchair. • Apply Vaseline or A & D ointment to dry skin in accordance with established policies. • Move and turn bedfast residents at least every two hours and record such information on CNA notes. • Observe disoriented and comatose residents and notify Charge Nurse immediately. • Apply postural support properly. He/she provides direct communication with the Director and/or House Supervisor of unanticipated patient care events and/or risk management activities within patient care unit(s); Utilizes nursing personnel effectively, assisting with required scheduling, staffing and assignments; Provides direction, supervision and evaluation of staff and/or staff education as necessary; Assists with performance evaluations, progressive discipline of staff and other related HR activities as needed; Assesses supplies and equipment availability, ensures their proper use and functionality and assists in the daily maintenance of par levels; Collaborates with other department managers/supervisors to ensure smooth coordination of services to patients; enhanced patient flow within the facility and efficient admission/discharge/transfer processes; Assesses patient care delivery and quality of care/documentation on a consistent basis; directs staff members to make immediate changes in the plan of care, as appropriate to ensure patient safety and adherence to regulatory guidelines, hospital policies, procedures, protocols and/or clinical pathways and notifies Department Director, House Supervisor and/or Medical Staff as appropriate utilizing the chain of command. Interviews patients/families on admission and during rounds to enhance positive patient experiences, provide comprehensive orientation to the patient care unit and increase patient/family awareness on how to access services and/or the charge nurse, if necessary. The Charge Nurse may perform direct patient care within scope of practice and clinical competency to ensure patient safety and patient/nurse ratios; he/she shall be temporarily relieved of Charge Nurse duties during such times by Director/designee and shall document details of same on patient care assignment record. Qualifications EDUCATION, EXPERIENCE, TRAINING 1. Current and valid state RN License 2. Current BLS upon hire and maintain current. 3. Previous supervisory experience, preferred. 4. Working knowledge of Mental Health Laws, Practice Standards, interdisciplinary treatment team process, behavior health programming and daily nursing functions for inpatient and outpatient behavioral units. 5. Continuum of care for behavioral health care, preferred. 6. Receive/ maintain LPS Certification as deemed necessary. 7. Must obtain approved training on LPS statuses and patient rights and successfully pass LPS Certification exam. 8. Must have approved LPS certification prior to writing 5150 holds in designated Prime facilities. 9. LPS Professionals: Must have experience in an inpatient mental health facility per County requirements. 10. Patients Rights Education/ Competency required 30 days upon hire and maintain current. 11. Bachelor of Science in Nursing degree, preferred. SUMMARY Plans, coordinates and manages the nursing department. Responsible for the overall direction, coordination and evaluation of nursing care and services provided to the residents. Maintains for each level of nursing personnel the nursing services philosophy and objectives, standards and practice, policies and procedures, and job descriptions. Provides quality care that is consistent with company and regulatory standards. May be designated to assume responsibilities of daily operations of the facility in the Administrator’s absence.
