Patients usually cared for in this area include those with trauma tic brain injuries, spinal cord injuries, rib fractures and lung injuries, as well as diverse orthopedic injuries requiring frequent monitoring and care. Patients may also be transferred to the TSDU from the Trauma /Surgical ICU when their clinical condition has improved but close monitoring or ventilator support is still required . The unit is staffed with registered nurses, critical care technicians, trauma surgery nurse practitioners and resident physicians. Staff also work closely with respiratory therapy and specialized surgical groups such as orthopedics and neurosurgery. Nurses and critical care technicians working in this area receive education specific to the care of the trauma patient. Serenity Hospice is looking to hire 'Registered Nurse (RN) Case Managers to join our team of professionals. RN Case Manager will provide service in the York County PA area. The RN Hospice Case Manager will be responsible for assisting the Team with Case Management/Visits to aid in coverage and call outs. Job duties can include, but are not limited to: Completing an initial assessment of patient and family to determine hospice care needs Regularly re-evaluates the patients' nursing needs Use assessment data to determine care plan needs Collaborate with the entire healthcare team, patient and family, to help direct the work of the team to provide are that is continuous and well-coordinated Counsels the patient and family in meeting end of life needs Initiates appropriate preventative nursing procedures Communicates with the physician regarding patient's needs and reports any changes in the patient's condition Supervises the Hospice Aides and LPN's The proper candidate must have a current Pennsylvania RN License in good standing, must be familiar with and willing to travel the area and have hospice experience. Must also have reliable transportation and insurance. Monday through Friday 8:30am to 4;30pm. Care for Hospice patients in their homes and facilities.
