JOB DESCRIPTION SUMMARY The hospice aide works under the supervision of a registered nurse and performs various tasks as assigned in accordance with the patient’s plan of care to meet the patient's personal needs and to promote comfort. The hospice aide is responsible for reporting and documenting observations and care performed. The hospice aide will be assigned in a manner that promotes quality, continuity and safety of a patient's care. ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES Responsibilities of the hospice aide include, but are not limited to, the following: 1. Performing personal care needs after given a patient's assignment and attending to his/her requests promptly. If unable to perform a certain task, report to the Case Manager immediately. 2. Providing patient and family with positive communication techniques, within realm of ethical and respectful care, including confidentiality. 3. Meeting safety needs of patient and using equipment safely and properly (foot stools, side rails, O2 etc.) 4. Giving personal care including baths, back rubs, oral hygiene, shampoos and changing bed linen as often as assigned. 5. Assisting in dressing and undressing patients as assigned. 6. Assisting in feeding the patient as assigned. Hospice Aides may not provide patients with homemade food items or baked goods that they themselves prepared 7. Providing proper care and observation of patient's skin to prevent breakdown of tissue over bony prominence. 8. Reporting any complaint of pain or other symptoms or changes in the patient's condition to the RN Case Manager. Provide normal range of motion and transfers/positioning as directed. 11. Assistance with self-administered medications as allowed by state regulations, such as “reminders.” 12. Offering and assisting with bedpans and urinals. Providing assistance as assigned with light laundry needs, emptying waste baskets and other light homemaker tasks. 13. Performing range of motion and other simple procedures as ordered with appropriate completed/demonstrated skills competency. 14. Providing respite for patient's/family/caregiver when on-site, as appropriate. 15. Keeping patient's living area clean and orderly, as assigned. 16. Adhering to the organization's documentation and care procedures and standards of personal and professional conduct. 17. Participates in quality assessment performance improvement teams and activities. 18. Respectful of patient and family/caregiver environment and patient's personal needs. 19. As a mandatory reporter, reports failure to comply with the requirements of chapters 246- 335 WAC and 70.127 RCW to the Washington Dept. of Health as required within 14 calendar days, using Dept. of Health forms. 20. Reports suspected abandonment, abuse, financial exploitation, or neglect of a person in violation of RCW 74.34.020 or 26.44.030 to the department of social and health services and the proper law enforcement agency. If you are comfortable with a non-routine, varied schedule with numerous clients each week and are the "best of the best" providing care and companionship to older adults, we have a rewarding positon for you. The Lead Caregiver provides crucial support to ensure our clients have the care they need if their regularly scheduled caregiver is sick or unavailable, and also ensures new clients are able to get care started without delay. In addition to providing care to clients, the Lead Caregiver assists with on-the-job training and peer mentoring and development of new members of our care team. The Lead Caregiver must be available for assignments any time within a 13 hour shift, either days or nights. Weekends are not required but pay a premium hourly rate. Travel up to 45 minutes from our office in Kettering is required, so a valid driver's license, vehicle, and car insurance is a must. Full time Lead Caregivers working 5 days/week are guaranteed pay for 40 hours per week minimum even if they are not placed in assignments for 40 hours. Part time Lead Caregivers working 3 days/week are guaranteed pay for 24 hours per week minimum even if they are not placed in assignments for 24 hours. Lead Caregivers receive premium pay in a range from $18/hr up to $20/hr, plus $3/hr additional on weekends. Position Overview: The Director of Nursing Services (DNS) plans, coordinates, and provides total nursing care for residents and provides supervision and guidance to clinical care team members. Key Responsibilities: Assume responsibility of the community in the absence of the Executive Director. Implement policies/procedures with follow-up and supervision of care team members to ensure compliance; work closely with Medical Director, Health Department, CDC, etc. to ensure current standard practices are in place; recommend to regional office changes in policies/ procedures as needed. Assist in preparing budget for the department; administer approved budget. Direct and delegate management of professional and ancillary nursing care team members. Coordinate activities of various nursing units/wings promoting and maintaining harmonious relationships among nursing personnel and with medical staff, residents, community staff and the public; screen clients prior to admission to ensure the center can provide services. Analyze and evaluate nursing and related services rendered to improve quality of resident care and plan better utilization of care team member time and activities. Ensure positive outcomes are accomplished for residents; responsible to develop systems and work schedules that respect resident’s lifestyle and ensure the resident reaches the highest attainable level of functioning; make rounds at least daily; follow up daily on each shift to ensure changes in status are reported and recorded. Ensure a Continuous Quality Improvement Program is in place and that appropriate follow-up occurs. Ensure new care team members are effectively oriented; ensure an ongoing educational program is in place. Develop goals and objectives for the department; motivate and manage care team members to accomplish the same. Qualifications: Graduate of accredited school of nursing. Licensed as a RN in the state employed; license must be active, valid, and in good standing. Must hold an active CPR license or ability to obtain within the first 30-days of hire; CPR license must be kept active and current throughout employment. Valid CPR teaching certificate preferred, but not required. Three year of nursing experience, of which one year in a long-term care environment. Must have at least two years supervisory experience.
