Nurse Practitioner, Longevity Care in Senior Living (In-Person Contract)

About Total Life Total Life connects adults 65+ with licensed clinicians for behavioral health, geriatric care and longevity services, covered by Medicare, Railroad Medicare and major commercial insurers. Since 2021 we've grown to more than 200 credentialed providers serving patients in 49 states. Most longevity medicine is built for healthy 40-year-olds. Ours is built for older adults. The Longevity Program combines a comprehensive geriatric assessment with chronic disease management, cognitive health, medication review and weight management, organized around five pillars of healthy aging: sleep, nutrition, movement, mental health and social connection. It works alongside a patient's existing care and never replaces their primary care provider. The role This is an in-person contract role. We're looking for nurse practitioners in the Tacoma-South Seattle area to lead longevity days at our partner senior living communities. Each day opens with a short educational talk for residents on healthy aging, using a presentation and speaker notes we provide. You then see the residents who sign up for one-on-one longevity visits. You'll be trained and supported by our Chief Medical Officer, a board-certified geriatrician who designed the program's clinical content. Your first site day is run alongside a member of our team. Schedule Site days are on weekdays. The presentation is the fixed part of the day. The rest depends on how many residents sign up. Expect roughly 4 to 8 hours per site day, a few times a month to start, with hours growing as bookings grow. Since 2021 we've grown to more than 200 credentialed providers serving patients in 49 states. Most longevity medicine is built for healthy 40-year-olds. Ours is built for older adults. The Longevity Program combines a comprehensive geriatric assessment with chronic disease management, cognitive health, medication review and weight management, organized around five pillars of healthy aging: sleep, nutrition, movement, mental health and social connection. It works alongside a patient's existing care and never replaces their primary care provider. The role This is an in-person contract role. We're looking for nurse practitioners in the Tacoma-South Seattle area to lead longevity days at our partner senior living communities. Each day opens with a short educational talk for residents on healthy aging, using a presentation and speaker notes we provide. You then see the residents who sign up for one-on-one longevity visits. You'll be trained and supported by our Chief Medical Officer, a board-certified geriatrician who designed the program's clinical content. Your first site day is run alongside a member of our team. Swedish Medical Group is seeking a dedicated Advanced Practice Clinician (Physician Assistant or Nurse Practitioner) to join the Critical Care team to support the APC program at Swedish First Hill and Cherry Hill campuses in Seattle, Washington. Position Details: Full-time position (1.0 FTE) 13 shifts per month (156 shifts per year); 7 a.m. to 7 p.m., variable schedule, includes weekends and holidays Provide comprehensive evaluation, diagnosis and management of critically/acutely ill patients Collaborate closely with critical care physicians and multidisciplinary teams to ensure high-quality, coordinated patient care Perform inpatient procedures consistent with training and scope of practice Compensation & Benefits: Compensation is between $145,767 and $190,261 per year Compensation may include productivity, quality and service incentives, or extra shift incentives as applicable to the position. In addition, providers may have CME benefits available.
