RNs oversee practical nurses, CNAs and nursing personnel through assessment of their knowledge, skill and capabilities, clarifying expectations and addressing behaviors to ensure the safety of residents and to attain the ultimate goal of achieving and maintaining the highest practicable physical, mental and psycho-social well-being of each resident. RESPONSIBILITIES Conducts Comprehensive Health Assessments and Care Planning: Gathers data from various sources to assess resident health, creates individualized Plans of Care, and ensures team collaboration in planning and execution. Implements and Monitors Nursing Care: Oversees the delivery of nursing interventions, evaluates outcomes, and continuously updates care plans to enhance resident well-being and quality of life. Ensures Compliance with Standards and Resident Rights: Delivers care aligned with Minnesota Board of Nursing standards, HIPAA, and the Residents’ Bill of Rights, promoting dignity, privacy, and equality in treatment. Provides Direct and Specialized Nursing Care: Performs RN-specific tasks such as IV administration, PICC line care, and resident education, while recognizing and addressing scope-of-practice limitations. Maintains Safety, Documentation, and Infection Control: Ensures a clean and safe environment, follows safety protocols, accurately documents care and medications, and upholds HIPAA and infection control standards. This role helps collect resident data, contributes to assessments such as MDS and RAPS, develops and updates comprehensive care plans, and collaborates with the interdisciplinary team to ensure resident needs are met. The LPN Care Coordinator monitors resident conditions, reports changes to RNs, participates in care conferences, and helps ensure care plans reflect current physician orders. Additional responsibilities include providing nursing care under RN supervision, supporting and directing LPNs and nursing assistants, assisting with staffing needs, and ensuring safe, competent care delivery. The position maintains accurate documentation, communicates care needs effectively, adheres to nursing policies and regulatory requirements, and supports quality improvement initiatives. The LPN Care Coordinator also maintains licensure, participates in ongoing education, and promotes a positive, inclusive, customer-focused workplace culture. Qualifications: Licensure and Certification Requirements : Must hold a current LPN license in Minnesota and complete CPR/emergency care certification; must meet employment eligibility per MN Department of Human Services background check. This role oversees occupational health and staff development functions, including infection control, employee injury management, worker health programs, and resident and employee safety. The ADON conducts surveillance, maintains records, evaluates incident reports, and provides data and recommendations to the Safety, Infection Control, and Quality Assessment/Assurance Committees.
