Responsibilities: • Assumes responsibility for all assigned patient care by effectively executing each patient’s Plan of Care. • Collaborates with the patient, family/caregiver(s), ordering physicians, other care team members, and community resources. • Submit accurate, complete, timely, meaningful, and required documentation, including progress and clinical notes. • Participate when possible in case conferences with other clinicians by reviewing services, clinical documentation, and discussing pertinent patient care issues and goals. • Participate in on-call rotation as reasonably assigned. Qualifications: • Minimum of 2 years nursing experience with a preference for 2+ years in an acute or a post-acute care setting. • Active State Registered Nurse licensure is required. • Proof of current CPR certification and valid Driver’s license.
