Hospice RN Case Manager

Duties: What you will do: Provide skilled nursing care Provide case management services Conduct initial assessments of new patients Travel to patients wherever they reside: Home, nursing facility etc. Participate in an on-call schedule Responsibilities you will have: Completes an initial comprehensive and ongoing comprehensive assessment of patient and family to determine hospice needs. Provides a complete physical assessment and history of current previous illness. Assess and evaluates a patient's status by: Writing and initiating the plan of care Regularly re-evaluating patient and family needs Participating in revising the plan of care as necessary Initiates the plan of care and makes necessary revisions as patient's status and needs change Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process. Counsels the patient and family in meeting nursing and related needs. Assume responsibility to coordinate patient care for assigned caseload. Completes and submits accurate and relevant clinical notes regarding the patient's condition and caregiving. How you make a difference The Charge Registered Nurse is responsible for overseeing the total health care delivery system in their assigned nursing unit, including the collection and evaluation of health status data, nursing diagnosis, goal setting, planning, implementation, and evaluation of patient care. They provide direct supervision of patient teaching and ensure that ancillary personnel only perform services for which they are qualified. The Charge RN is accountable for the overall quality of patient care in their assigned unit. Key Responsibilities Supervising the total health care delivery system in their assigned unit, ensuring compliance with policy and procedure, and coordinating patient care with other departments. Monitoring nursing service, maintain accurate records, and identify potential problems, proposing corrective actions as needed. Assisting with staff orientation, training, and evaluations, and coordinate staffing levels to meet contractual requirements. Ensuring the cleanliness, maintenance, and appearance of the unit and maintain appropriate levels of medical equipment and supplies. Maintaining a professional atmosphere and attend monthly staff meetings as directed by the Director of Nursing or Administrator. Qualifications & Requirements Education Graduate of an accredited School of Nursing. Experience Prefer a minimum of one-year clinical experience. Licenses/Certifications Current licensure as a Registered Nurse within the state of employment. Current CPR Certification. Assists in medical or minor surgical procedures, implements nutrition and therapeutic diet plans, and documents nursing encounters using SOAP charting, ensuring effective communication with nursing staff, physicians, and other relevant personnel. Attends mandatory staff meetings and training sessions, demonstrating a commitment to continuous professional development and adherence to policies and procedures in providing high-quality patient care. Qualifications & Requirements Education Graduate from an accredited school of nursing.
