The Clinical Assessor / Coordinator, LPN identifies actual and potential mental and physical health problems and accurately determines the necessary classification and regimes to assist with the appropriate placement of a potential admission to the Gurwin Jewish Health Care System. HOURS: Monday through Friday 9:00am-5:00pm EDUCATION AND EXPERIENCE REQUIREMENTS Licensed to practice as LPN in New York State.
