Description A nurse practitioner (NP) as described in Title VIII, Article 139 section 6910 of the New York Education Law is a RN who has earned a separate certification as an NP through advanced clinical nursing education in a distinct specialty area of practice. Nurse practitioners are independent practitioners who diagnose, treat, and prescribe for a patient’s condition that falls within their specialty area of practice. NOTE: Medical orders, progress notes and treatments plans are not required to be countersigned by a physician. Responsibilities CLINICAL PRACTICE 1. Assesses the health status of patients/families by means of comprehensive health history and physical examination. 2. Initiates and orders selected therapeutic and diagnostic procedures, consultations and laboratory examination, prescribes medication regimens, interprets findings and alters plan of care according to accepted standards of care and practice guidelines 3. Educates and counsels in the areas of health promotion, maintenance, disease prevention, and management of acute/chronic illnesses. 4. Documents findings and plan of care in the patient's medical record. 5. Consults with physician according to established standards, policies, procedures, and protocols. 6. Demonstrates the knowledge and skills necessary to provide care, based on physical, psychosocial, educational, safety, and related criteria, appropriate to the age of the patients served in assigned area. 7. Initiates referrals to specialty services and fosters continuity of care. 8. Participates in team meetings and conferences to enhance an interdisciplinary approach to care delivery. 9. Maintains patient/employee confidentiality in the management of information. 10. Develops the plan of care based on the diagnoses and individual patient needs: • Records, reports, and interprets the client’s responses to the plan of care. 11. Performs a patient record review with the collaborating physician in a timely fashion but not less than every three months if utilizing a collaborative practice agreement. 12. Completes the Focused Professional Practice Proctoring Evaluation (FPPE) or the Ongoing Professional Practice Proctoring Evaluation (OPPE) as required by Medical Staff for continued credentialing status. 13. Practices as a member of the allied health staff according to the rules and regulations of the Medical Staff and the bylaws of the Hospital Staff. PROFESSIONAL DEVELOPMENT 1. Supports the mission, vision, philosophy and goals of the Department of Nursing and the MSHS. 2. Demonstrates accountability for own nursing practice. 3. Advocates for patients/families and supports patients' rights. 4. Considers ethical issues of professional nursing practice and adheres to the code of ethics. 5. Participates in defining, maintaining, and interpreting standards of nursing practice. 12. Contributes to the maintenance of a positive image of advanced practice nursing in the medical center and in the local community by implementing programs which recognize nursing as a profession. 13. Participates in departmental and interdisciplinary committees which influence and/or determine policies affecting nursing practice and patient care delivery. 14. Seeks opportunities to enhance professional nursing practice and influences outcomes. 15. Treats co-workers, patients and families with dignity and respect. Excellent interpersonal skills, effective communication skills, creative problem solving and excellent critical thinking and leadership Qualifications EDUCATION REQUIREMENTS Licensure: New York State License with current registration as a registered nurse. State Certification: Certified by the New York State Department of Education as a Nurse Practitioner with current registration as follows: Is a graduate of New York State registered Nurse Practitioner Program or equivalent OR is certified as a Nurse Practitioner by a national association via examination. Has completed a pharmacy component of not less than the required semester hours or the equivalent AND has been instructed in New York State and Federal Laws relating to prescriptions and recordkeeping (3-hour course). Has a Collaborative Practice Agreement with a physician and designated protocols if less than 3600 hours experience as a practicing Nurse Practitioner. OR Has a Collaborative Relationship with a physician based on employment in clinical practice in the NP role at Mount Sinai Hospital. If there is a documented work history as a Nurse Practitioner with greater than 3600 practice hours. Please note all NPs regardless of their status will be placed on an FPPE initially) Education: Master's degree program designed to prepare a NP to diagnose, treat, and prescribe for a patient’s condition that fall within their specialty area of practice or the successful completion of an approved supplemental education program, i.e., post-Masters NP certificate. Experience: Previous progressive experience relevant to the area of clinical practice required. For all NPs hired effective June 1, 2019, National Board Certification in Advanced Practice Nursing required. For NPs who bill for professional services, National Board Certification is required regardless of hire date. The Board Certification requirement may be deferred up to 1 year if approved by Chief Nursing Officer or designee. RESPONSIBILITIES: Completes comprehensive assessments of patient and family to determine home care needs at regular established timelines. Observe ethical and legal guidelines for safeguarding confidentiality of patients. Assess patient needs and physical status each skilled visit through health data assessment and patient interview. Re-evaluate patient needs through physical assessment, response to therapy and any updated physician’s orders. Inform physician of any changes in patient condition, response to care, progress etc. Obtain and clarify orders for Plan of Treatment revisions. Prepares clinical notes and updates the primary physician when necessary and at least every sixty days. Completes And submits all documentation in accordance with policy and procedures. Communicates with the physician regarding the patient’s needs and reports and changes in the patient’s condition; obtains/receives physician’s orders as required. Communicates with other health related persons to coordinate care plan. Participates in on-call duties as defined by the on-call policy. Ensures that arrangements for equipment and other necessary supplies and services are available. Instructs, supervises and evaluates home health aide/personal care aide care provided every 14 days or as stipulated by law and regulation. Instructs, supervises and evaluated LPN/LVN care provided at least every 30-days. Exercises professional management. And coordinates care between and among involved parties in home, community providers and pediatric tertiary specialists. QUALIFICATIONS: Bachelor's degree of Nursing from an accredited college/university Current active and unrestricted nursing license Pediatric care experience in the acute and/or home care environment Valid driver’s license Current certification in Basic Life Support for Health Care Professionals.
