Description The (Per Diem) Clinical Nurse is a Registered Professional Nurse who provides safe, competent quality care based on nursing theory and research to a designated group of patients and significant others. Responsibilities Patient Care 1. Delivers patient care by incorporating the tenets of the professional practice model of Relationship Centered Care and through the application of the nursing process based upon theory, research, evidence-based practice, and approved organizational nursing standards. 2. Overview Oversees, supervises, coordinates, facilitates, and supports the Hospice After Hours care delivery activities, including field staff supervision. Manages continuity of care to all Hospice patients and their families after normal operating hours. Responds to incoming calls from patients/families/caregivers, assesses and priorities patient needs based on policies and procedures and staff capacity, ensuring all emergent and urgent request are met timely. Addresses needs within context of patient's Interdisciplinary Plan of Care and Hospice Care mission, objectives, policies, and procedures. Documents and reports to team patient's needs, significant events, plan of care and other issues. Evaluates effectiveness of clinical directions. Provides support and direction to families in accordance with the patient’s Interdisciplinary Plan of Care and Hospice Care mission, objectives, policies, and procedures. • Takes referral information and triages for immediate response, dispatches the on-call nurse to do the evaluation/ admission. Contacts Physician for verification of prognosis and appropriateness for Hospice/Palliative care. Contacts and interface with team members, primary physician, team physician, administrator, and other staff to ensure patient’s and family’s needs are met. • Works directly with outside vendors and care delivery partners, to ensure when patients are transferred to General Inpatient Facilities, Emergency Departments, and Paramedicine Activation, that documents and patient information to ensure continuity of care and most effective care based on patient/caregiver goals and clinical needs. • Provides clinical directions to patients/caregivers in accordance with Hospice guidelines. Obtains and receives verbal orders from team/physician and documents care using standard form. Evaluates and documents effectiveness of directions through telephone follow-up with patients/caregivers. Provides team/physician with documentation and implements any changes, as directed. • Obtains and reviews daily report from each team on significant patient care situations that may require contact/follow-up during course of shift. Documents all calls received using high degree of accuracy and detail to ensure team/physician is provided with all relevant and necessary information. Completes case conference form on all calls received and faxes to respective team. Supervises on-call Nurses: Monitors/evaluates performance, provides guidance, recommends hiring and termination, as needed. Description Job Title - Remote Nurse Practitioner, Palliative Care at Home - Mount Sinai Home Health - Full Time - Day Palliative Care at Home, part of Mount Sinai at Home, provides home based palliative symptom assessment, treatment and ongoing goals of care discussions for patients and families facing advanced illness. The full clinical team is comprised of CHWs, NPs, nurses, social workers and physicians. The team works in collaboration with patients’ healthcare teams to provide high quality evidence based complex palliative care and chronic care management and is designed to improve symptoms and QOL, reduce caregiver burden, reduce unnecessary inpatient utilization, and facilitate hospice when appropriate. Operational Duties • Conducts in home and telehealth assessments for patients with complex needs, including formulating a diagnosis, ordering and interpreting diagnostics, and delivering appropriate care in line with patient preferences and goals of care • Prescribe new medications and perform medication reconciliation • Demonstrate sound clinical judgement and evidenced based practice in symptom management and end of life care • Facilitate family meetings, goals of care conversations, and ACP discussions • Leads oversight of care plans in conjunction with the rest of the clinical care team • Advocates for the patient and caregiver in collaboration with other care providers to keep care coordinated and consistent with patient goals • Serve as a link between health and social care services and community to improve access to palliative care and quality of care • Work closely with interdisciplinary team to address changes in patient needs or symptoms • Educates patient/family on trajectory of illness, use of medications, assesses disease understanding • Participate in weekly IDT meeting with care team • Excellent teamwork, communication and leadership skills • Documentation as required • Performs other related duties as assigned Qualifications Required: Master's degree program designed to prepare a NP to diagnose, treat, and prescribe for a patients condition that fall within their specialty area of practice or the successful completion of an approved supplemental education program, i.e., post-master's NP certificate. • Licensure: New York State License with current registration as a registered nurse. • State Certification: Certified by the New York State Department of Education as a Nurse Practitioner with current registration as follows: • a. Is a graduate of New York State registered Nurse Practitioner Program or equivalent OR is certified as a Nurse Practitioner by a national association via examination. • b. Has completed a pharmacy component of not less than the required semester hours or the equivalent AND has been instructed in New York State and Federal Laws relating to prescriptions and recordkeeping (3-hour course). • c. Has a Collaborative Practice Agreement with a physician and designated protocols if less than 3600 hours experience as a practicing Nurse Practitioner. OR • Has a Collaborative Relationship with a physician based on employment in clinical practice in the NP role at Mount Sinai Hospital. If there is a documented work history as a Nurse Practitioner with greater than 3600 practice hours. Please note all NPs regardless of their status will be placed on an FPPE initially) Experience: Previous progressive experience relevant to the area of clinical practice required. For all NPs hired effective June 1, 2019, National Board Certification in Advanced Practice Nursing required. For NPs who bill for professional services, National Board Certification is required regardless of hire date. The Board Certification requirement may be deferred up to 1 year if approved by Chief Nursing Officer or designee. Compensation The Mount Sinai Health System (MSHS) provides salary ranges that comply with the New York City Law on Salary Transparency in Job Advertisements. The salary range for the role is $123576 - $197400 Annually. Actual salaries depend on a variety of factors, including experience, education, and operational need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
