What are we looking for? Current RN license in assigned state is required; Bachelor degree in nursing preferred. Minimum of 6-8 years nursing experience. Certified Case Manager (CCM) required, or willingness to obtain within 12 months of hire, unless candidate has 2-3 years of relevant care/case management experience 2+ years experience in transitional nursing, emergency room nursing, care coordination, discharge planning, or home health is strongly preferred. Demonstrated skill in motivational interviewing, patient activation, time management, and navigating community and social resources. Responsibilities* Role Under physician supervision, coordinates care of acutely ill post-surgical Adult/Late Adult patients admitted to the Cardiac Surgery Service intensive care setting. Specifically, role requires clinician to: Perform advanced diagnostic and therapeutic tasks associated with patients undergoing Cardiac, Vascular and Thoracic procedures, including heart and lung transplantation and ventricular assist devices. Collect detailed histories and physicals and present pertinent data to the medical staff. Collect and interpret reports from diagnostic lab tests, radiographic procedures, and clinical studies. Order tests and prescribe medications. Perform various diagnostic, therapeutic, and minor procedures, based on privileges. Assist attending physician in orientation of new resident physicians to the service and assist in maintaining continuity of care throughout the Cardiovascular center. Confer with attending physicians and communicate with other care providers/consultants to coordinate clinical care. Maintain accurate documentation regarding patient status, evaluations, and treatment. Streamline the patient transfer process between levels of care, including writing discharge orders and communicating specific patient needs to team members. Participate in quality assurance activities, peer evaluations and support research initiatives involving CVICU and cardiac surgery patients. Comply with established standards for quality, patient safety and re-imbursement. Perform other related duties and initiatives as deemed necessary in the best interests of patient care. Utilize new improvements and/or technologies that relate to job assignment. Note: Position is night/day rotator with the majority of shifts being on nights. Holiday and weekend rotation required, weekend requirement could be as much as 50% or every other. Required Qualifications* Np Current licensure to practice as a Registered Nurse and Nurse Practitioner specialty certification in the State of Michigan. Current specialty certification to practice as an Adult-Gerontology Acute Care/Primary Care Nurse Practitioner, and/or Family Nurse Practitioner with 2 or more years of ICU experience caring for critical care patients. Graduate of an accredited Nurse Practitioner program. Certification as a Nurse Practitioner recognized by the National Council of State Boards of Nursing (NCSBN). Recent clinical ICU experience in the last 5 years caring for acutely ill patients with diseases common in Cardiac and Thoracic Surgery. Current DEA registration required ACLS required (separate AHA approved BLS also required unless ACLS was completed through the UMHS Simulation Center). Pa Current license to practice as a Physician Assistant in the State of Michigan with 2 or more years of ICU experience caring for critical care patients. Satisfactory completion of a Physician Assistant program accredited by Accreditation Review Commission on Education for the Physician Assistant (ARC-PA) or one of its predecessor agencies, CAHEA or CAAHEP. Certification by the National Commission on Certification for Physician Assistants (NCCPA). Recent clinical ICU experience in the last 5 years caring for acutely ill patients with diseases common in Cardiac and Thoracic Surgery. State of Michigan Controlled Substance License Required Current DEA registration required ACLS required (separate AHA approved BLS also required unless ACLS was completed through the UMHS Simulation Center).
