Description The Medical Office Specialist offers clinical and medical office support to physicians and surgeons. Assists in patient care and administrative duties necessary to deliver quality patient care. Performs various duties as a member of the health team in the treatment and care of patient and their families and also assists with tasks to support the unit. Participates and supports the health team in the direct patient care under the supervision of an RN/LPN or MD. Responsibilities Monitors patient flow in waiting room and resolves patient flow issues related to registration and post-visit processing Provides set-up of examination room and instruments in accordance with proper techniques established by hospital and infection control policies. Takes and monitors vital signs, including fetal heart rate if necessary Performs phlebotomy, point of care testing, and other procedures as indicated by physician order, including but not limited to electrocardiogram and phototherapy Assists with procedures as necessary including but not limited to echocardiogram, sigmoidoscopy, proctoscopy, nasal scope, colposcopy, biopsy, LEEP, laser and cystoscopy Sets up oxygen and suction Performs vision screening Inventories, orders or re-stocks medical supplies; monitors expiration dates on all medications and medical supplies disposing appropriately as per institution policy Maintains logging system as required for refrigeration of medications, point of care testing, sterilization process, or others as needed Performs routine maintenance/cleaning of equipment and instruments; Notifies appropriate personnel regarding any malfunction in equipment Maintains a safe, secure, and healthy work environment by following standards and procedures; complies with legal regulations Complies with established personal protective equipment requirements necessary for protection against exposure to blood and other potentially infections body fluids, chemical disinfectants, radiation and other hazardous substances Acts rapidly and calmly during emergencies, initiates CPR and provides the necessary items called for by the team Transports patients and equipment as needed Performs any and all duties involving direct patient care as assigned by the RN/LPN within the framework of the New York State Nurse Practice Act and MSHS Policy and Procedures Documents in medical record / electronic medical record as required by hospital policy Greets and registers patients, prepares and processes appropriate forms, directs patients to appropriate areas and arranges transportation needs Screens incoming calls, takes and responds to messages, as well as responds to inquiries regarding department procedures, services and programs. Schedules appointments and completes pre-registration via telephone including insurance verification and full patient demographics collection over the telephone or in person using computerized system(s) with accountability for the integrity of the data, including all patient demographics and insurance information Fulfills clerical responsibilities as assigned using the electronic medical record (EMR) system or manually as necessary, to include sending and receiving patient medical records; obtaining lab/x-ray reports, discharge summaries, referral information, etc. Performs routine cash collections at time of service. Completes forms and requisitions as needed, managing charts to ensure information is completed and filed appropriately. Assists in the creation and maintenance of medical charts. Retrieves lab, pathology and diagnostic testing, and operative reports and data from computer systems Checks, verifies and updates insurance information utilizing internal or electronic sources. Obtains and documents managed care referrals, required pre-certifications and authorizations as necessary Supports surgical or procedure scheduling in conjunction with Surgical Coordinator or other similar role(s) verifying times with patients, preparing charts, pre-admissions and consent forms Maintains a working knowledge of New York State third party payer eligibility requirements for private and hospital billing Utilizes and understands basic medical terminology; able to complete visit encounter forms and enters ICD10-CM and CPT information into computerized systems as appropriate Assists in training, orientation and education of entry level and or/float staff Qualifications High School Diploma/GED. Medical Assistant Certification from an approved school required. Description Job Title - Remote Nurse Practitioner, Palliative Care at Home - Mount Sinai Home Health - Full Time - Day Palliative Care at Home, part of Mount Sinai at Home, provides home based palliative symptom assessment, treatment and ongoing goals of care discussions for patients and families facing advanced illness. The full clinical team is comprised of CHWs, NPs, nurses, social workers and physicians. The team works in collaboration with patients’ healthcare teams to provide high quality evidence based complex palliative care and chronic care management and is designed to improve symptoms and QOL, reduce caregiver burden, reduce unnecessary inpatient utilization, and facilitate hospice when appropriate. Nurse practitioners will be responsible for the symptom management of chronically/terminally ill patients within an interdisciplinary team environment. This role will provide disease process and trajectory education to the team, patients and families. The NP will assess, order and interpret clinical testing, diagnose and treat symptoms related to a variety of chronic disease states to focus on palliative symptoms and advanced illness education. Responsibilities DESCRIPTION OF COMPETENCIES A. Operational Duties • Conducts in home and telehealth assessments for patients with complex needs, including formulating a diagnosis, ordering and interpreting diagnostics, and delivering appropriate care in line with patient preferences and goals of care • Prescribe new medications and perform medication reconciliation • Demonstrate sound clinical judgement and evidenced based practice in symptom management and end of life care • Facilitate family meetings, goals of care conversations, and ACP discussions • Leads oversight of care plans in conjunction with the rest of the clinical care team • Advocates for the patient and caregiver in collaboration with other care providers to keep care coordinated and consistent with patient goals • Serve as a link between health and social care services and community to improve access to palliative care and quality of care • Work closely with interdisciplinary team to address changes in patient needs or symptoms • Educates patient/family on trajectory of illness, use of medications, assesses disease understanding • Participate in weekly IDT meeting with care team • Excellent teamwork, communication and leadership skills • Documentation as required • Performs other related duties as assigned Qualifications Required: Master's degree program designed to prepare a NP to diagnose, treat, and prescribe for a patients condition that fall within their specialty area of practice or the successful completion of an approved supplemental education program, i.e., post-master's NP certificate. • Licensure: New York State License with current registration as a registered nurse. • State Certification: Certified by the New York State Department of Education as a Nurse Practitioner with current registration as follows: • a. Is a graduate of New York State registered Nurse Practitioner Program or equivalent OR is certified as a Nurse Practitioner by a national association via examination. • b. Has completed a pharmacy component of not less than the required semester hours or the equivalent AND has been instructed in New York State and Federal Laws relating to prescriptions and recordkeeping (3-hour course). • c. Has a Collaborative Practice Agreement with a physician and designated protocols if less than 3600 hours experience as a practicing Nurse Practitioner. OR • Has a Collaborative Relationship with a physician based on employment in clinical practice in the NP role at Mount Sinai Hospital. If there is a documented work history as a Nurse Practitioner with greater than 3600 practice hours.
