Performs exceptional care by responding promptly to patient requests. Strives to anticipate patient needs and resolve them proactively. Teaches patients/families/caregivers about patient medical condition, patient status, treatment plan, medications and possible side effects, and follow-up measures. Translates complex medical terminology to ensure complete understanding. Teaches patients/families/caregivers about any non-medicinal follow-up measures, such as healthy diet and exercise, disease prevention, and/or other lifestyle changes. Prepares patients and families/caregivers for future self-management. Promotes and maintain optimal client care under the supervision of an RN. Maintains a safe patient environment. Identifies and communicates significant abnormalities in patients data to case manager and other appropriate home health team members. Administers prescribed medications and treatments as permitted by law. Assesses all medicines a client may be taking to identify possible ineffective drug therapy or adverse reactions, significant side effects, drug allergies, contraindicated medications and reports any problems to the physician. Re-evaluates client’s nursing needs and reports findings to Case Manager. Counsels and instructs the client and family in meeting nursing and related needs. Prepares clinical and progress notes. Submits all documentation in a timely manner in accordance with agency policy. Documentation on patients chart correlates with nursing care plan. Communicates frequently and works closely with other home care team members.
