Collaborate in team-based care within a Patient-Centered Medical Home (PCMH) to deliver coordinated, comprehensive, and personalized care that supports patient wellness and improves health outcomes. The coordinator uses data analytics and patient outreach to proactively manage patient populations and support quality metrics that align with financial incentives and regulatory goals. Qualifications: Education/ Experience: Certification per American Association of Medical Assistants preferred (AHA) or LNA License. Other Requirements: BLS, Proficient computer skills required. Experience working with an electronic health record preferred.
