They also provide functional support/supervision to Home Healthcare Aides and LPNs as needed and therapeutic intervention, overall care management and achievement of top decile client outcomes and patient satisfaction. Position Details: Schedule: Monday - Friday, 8 am - 5 pm, one weekend every 4 weeks and two rotating holidays per year. NO ON CALL REQUIREMENTS! Coverage Region: Enfield, Windsor Regions Bonus: Position qualifies for a $10,000 sign-on bonus paid out over two years. NOTE: This position is 100% home care providing services in patients’ private homes. What you will do: Plan and prioritize visits based on client needs and physician orders. Assess clients at admission and key intervals and document accurately. Develop and update care plans with measurable goals in collaboration with physician and client. Provide therapy using approved techniques, exercises, and modalities. Coordinate care and referrals with other disciplines as needed. Educate clients and caregivers on therapy programs and home exercises. Monitor progress and report changes promptly to the physician. Ensure compliance with laws, regulations, and agency policies. Complete documentation at point of care in a timely manner. The Value Analysis Specialist is responsible for the Product Evaluation/Value Analysis Program (PEVA) through collaboration with Executive Leadership, Supply Chain Leadership, Medical Staff, Clinical Staff, and Vendors. Honor resident right to refuse care, report such refusal to nurse supervisor. Assist in maintaining a safe, neat and clean environment. Answers resident call bells promptly and courteously. Ensure that residents who are unable to call for help are checked frequently. Takes and documents temperature, pulse and respirations rates, food and fluid intake and output, and heights and weights and records in EHR. Ambulates and transfers residents using the appropriate assistive devices and body mechanics. Use only the equipment you have been trained to use, operate in a safer manner and reports any defective equipment to supervisor immediately. Communicate and interacts effectively and tactfully with the residents, visitors, families, peers, and supervisors. Assist and escort residents to appointments such as the hair salon or attending activities, church services or doctor’s appointments. Our Certified Nursing Assistant – CNA (State Tested Nursing Assistant-STNA in Ohio) work with community management to provide seniors with personalized care, and give resident status updates at the beginning and end of each shift. Nursing assistants check in with residents, assist with dining and personal care needs, and perform vital sign checks and clinical procedures according to community policy. Provide assistance with activities of daily living (showers, bathroom assistance, dressing/grooming) Successful completion of State CNA/STNA course is required. Must maintain certification.
