Job Description Be a part of a world-class academic healthcare system at UChicago Medicine as a Clinic RN, Float in our MultiSpecialty clinic at our Crown Point location. In this role, the Multispecialty Clinic Nurse, Float (RN) is responsible for coordinating diagnostic and therapeutic regimens for a specified group of patients, and maintains cohesiveness between clinical research, physicians, and patients. The Nurse Associate coordinates physician activities with referring physicians, nursing staff, hospital administration, ancillary services, support services, insurance companies and other departments/sections and agencies as needed and assists the physician with a wide range of activities necessary to manage clinical research programs. In addition, the Nurse Associate functions as a liaison between physicians, patients, and families, assists physicians with bedside procedures, and provides patient care in the clinic with the physician as necessary. Essential Job Functions: Conducts, responds to, follow up on, and triages patient phone calls to the physician as appropriate. Floats within multispecialty ambulatory clinic to assists physician in clinic or procedure area with patient examinations, diagnostic tests and/or therapeutic procedures. Counsels patients concerning lifestyle modifications, medication regimens, side effects, and interactions using an appropriate psychosocial communication level. Communicates to clinics coordinators follow-up appointments, ancillary and/or pre-operative testing and scheduling needed for patient. Arranges patient admissions; coordinates ordering of tests/procedures with fellows, attending, and other nursing staff. Assists physicians in coordination of research programs by screening patients for acceptance into clinical research and outcome programs. Maintains office file notes, reviews charts, enters data for sectional demographic data base, assists in summarization and analysis of data. Required Qualifications: Registered Nurse, in good standing, licensed in the state of Indiana. Minimum of 3 years’ experience in a hospital progressive care or intensive care unit or equivalent experience. Outpatient RN experience preferred. The Home Health Intake & Referral Nurse Liaison (RN) works closely with hospital discharge planners and meets with patients and families to facilitate the home health admission process. Essential Job Responsibilities Completes an initial assessment of patient and family to determine home health care needs. Provides a complete physical assessment and history of current and previous illness(es). Educates the patient and family (caregiver) in ways to achieve home health goals. Provides health care instructions to the patient, family (caregiver) as appropriate per assessment and/or plan. Provides direct patient care as needed. Documents the initial patient and family assessments and interventions. Performs initial communication with the physician regarding the patient’s needs. Ensures that arrangements for equipment and supplies are made. Customer/Referral Source Management: Primarily works with designated hospital and other potential referral sources to develop and promote awareness of home health care services, discuss the agency’s capabilities, and educate on the services provided by home health. Facilitates communication between the referral source and the home health department. Works closely with clinical staff to ensure that necessary services are provided. This position will report to our client in Olympia Fields, IL. This opportunity would be part-time on a PM shift (5pm - 9:30pm). T his part-time position has shifts available Monday through Sunday. The expectation is that the ideal candidates would be able to pick up 3-to-5 PM shifts per week. There will be a weekend differential in pay for those that decide to pick up a weekend shift. Provides a complete physical assessment and history of current and previous illness(es). Regularly re-evaluates referral for patient/family hospice needs. Initiates the plan of care as needed.
